医护人员携带金黄色葡萄球菌耐消毒剂基因检测及消毒剂抗性研究

Detection of disinfectant-resistant genes of Staphylococcus aureus carried by medical staff and disinfectant resistance research

目的了解医务人员携带金黄色葡萄球菌耐消毒剂基因及其对消毒剂抗性情况,为科学有效地消毒提供参考依据。方法采用鼻拭子采样和基因分析方法,对某医院部分医务人员和健康体检人员携带金黄色葡萄球菌耐消毒剂基因及消毒剂抗性进行评价。结果共采集110名医护人员和336名健康体检人员鼻腔拭子,共分离出金黄色葡萄球菌125株和凝固酶阴性葡萄球菌333株。医务人员和健康体检人员携带的金黄色葡萄球菌qac A/B、smr基因检出率分别为39. 0%、17. 1%和13. 1%、3. 6%;携带凝固酶阴性葡萄球菌qac A/B、smr基因检出率分别为22. 0%、15. 1%和8. 3%、6. 3%。苯扎溴铵对携带qac A/B基因的葡萄球菌和凝固酶阴性葡萄球菌MIC值均> 4mg/L。结论医护人员携带金黄色葡萄球菌耐消毒剂基因检出率均明显高于健康体检人群,提示医务人员应加强手卫生消毒措施。

Objective To investigate the disinfectant-resistant genes of Staphylococcus aureus( S. aureus) carried by medical staff and their resistance to disinfectants,so as to provide a reference for scientific and effective disinfection. Methods Using nasal swab sampling and genetic analysis methods,the disinfectant-resistant genes and disinfectant resistance of S. aureus carried by some medical staff and health examiners in a hospital were evaluated. Results Nasal swab samples of 110 medical staff and 336 health examiners were collected,and a total of 125 strains of S. aureus and 333 strains of coagulase-negative staphylococci( CONs) were isolated. The detection rates of the qac A/B and smr genes of S. aureus carried by medical staff and health examiners were 39. 0%,17. 1% and 13. 1%,3. 6%,respectively;the detection rates of qac A/B and smr genes of CONs were 22. 0%,15. 1% and 8. 3%,6. 3%,respectively. The MIC value of benzalkonium bromide to staphylococci carrying qac A/B gene and CONs were both > 4 mg/L. Conclusion The detection rate of disinfectant-resistant genes of S. aureus carried by medical staff is significantly higher than that of health examiners,which suggests that medical staff should strengthen hand hygiene and disinfection measures.
