从心脾积热论治口疮验案举隅

Case Study of Differentiating and Treating Aphtha from Heart-spleen Accumulated Heat

口腔溃疡俗称“口疮”,是一种发病于口腔黏膜的疾病,多见于软腭、前庭沟、舌头、唇内侧、舌腹、颊黏膜部位,以上部位黏膜角化较差或缺乏角质层均会引起口疮。一旦发病患者疼痛剧烈,局部呈现明显灼痛,严重者还可能影响说话、饮食,影响日常生活,甚至并发口臭、便秘、恶心、慢性咽炎、发热、乏力、头晕、头痛、淋巴结.肿大等症状。当前西医治疗无特效药,在对症治疗基础上,以心脾积热为主,在治疗中着重清热解毒、泻心脾之火,同时还兼顾保护患者脾胃,以改善患者临床症状,进一步改善预后。

Oral ulcer,commonly known as aphtha,is a disease of oral mucosa.It is more common in soft palate,vestibular sulcus,tongue,inner lip,tongue ventrum and buccal mucosa.The poor mucosakeratinization or lack of stratum corneum at the above sites will cause aphtha.Once pain of onset patients is severe,the local will show obvious burning pain,the serious cases may influence their speech,diet and daily life,even accompanied with ozostomia,constipation,nausea,chronic pharyngitis,fever,fatigue,dizziness,headache,lymphadenectasis and other symptoms.At present,there is no specific medicine in western medicine treatment.On the basis of symptomatic treatment,heart-spleen accumulated heat is focused on.In treatment,heat-clearing and detoxicating is emphasized,heart-spleen fire is decreased.And at the same time,spleen and stomach of patients are protected to improve their clinical symptoms,and further improve prognosis.
