补肾颐年方治疗晚期前列腺癌经药物去势治疗后雄激素缺乏综合征疗效及对认知功能和不良情绪的影响

Effect of Using Bushen Yi'nian Recipe in the Treatment of Advanced Prostate Cancer after Castration with Andro-gen Deficiency Syndrome and Its Influence on Cognitive Function and Negative Emotion

目的:观察补肾颐年方治疗晚期前列腺癌经药物去势治疗后雄激素缺乏综合征(ADSACT)的疗效及对认知功能和不良情绪的影响。方法:将83例晚期列腺癌经药物去势治疗后ADSACT患者随机分为观察组(41例)和对照组(42例),对照组给予原药物去势治疗+对症处理。观察组在对照组基础上给予补肾颐年方治疗,疗程3个月。比较2组的临床疗效,记录2组治疗前后中医症状积分、男性更年期症状评分问卷(PADAM)评分、蒙特利尔认知功能评估量表(MoCA)评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分及血清睾酮、前列腺特异抗原(PSA)水平。结果:与治疗前比较,2组治疗后各单项中医症状积分和总分、PADAM量表中各单项评分与总分、HAMA评分、HAMD评分均明显降低(P<0.05),治疗后以上积分观察组较对照组改善更明显(P<0.05);与治疗前比较,观察组注意力、视空间与执行力、定向力、延迟回忆评分与总评分明显增加,且明显高于对照组(P<0.05),对照组治疗前后比较无差异无统计学意义(P>0.05);与治疗前比较,2组治疗后血清睾酮水平明显升高(P<0.05),PSA水平明显降低(P<0.05);治疗后观察组血清睾酮水平升高和PSA水平降低较对照组更明显(P<0..05)。观察组临床总有效率为87.8%(36/41),对照组为66.7%(28/42),观察组显著高于对照组(P<0.05)。结论:补肾颐年方治疗晚期前列腺癌经药物去势治疗后ADSACT的疗效确定,不仅能够改善患者临床症状,调节血清睾酮和PSA水平,还有助于认知功能的改善及焦虑抑郁不良情绪的缓解。

Objective:To observe the therapeutic effect of Bushen Yinian Recipe on androgen deficiency syndrome(ADSACT) and its influence on cognitive function and negative emotion in patients with advanced prostate cancer after castration.Methods:A tolal of 83 ADSACT patients with advanced adenocarcinoma after castration were randomly divided into two groups:an observation group(41 cases) and a controlled group(42 cases).The observation group was treated with Bushen Yi’nian Recipe on the basis of the controlled group for 3 months.The clinical effects of the two groups were compared,and the scores of TCM symptom score,male climacteric symptom score(PADAM),Montreal cognitive function assessment scale(MoCA),Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),serum testosterone and prostate specific antigen(PSA) were recorded before and after treatment.Results:Compared with before treatment,the scores of TCM symptoms and total scores,PADAM scores and total scores,HAMA scores and HAMD scores were significantly reduced(P<0.05).After treatment,the above integral in the observation group improved more significantly than that in the controlled group(P<0.05).Compared with before treatment,the scores of attention,visual space,executive ability,orientation,delayed recall and total scores in the observation group were significantly higher than those in the controlled group(P<0.05),and there was no significant difference between the two groups(P>0.05).Compared with before treatment,the level of serum testosterone and PSA in the two groups were significantly higher(P<0.05) and lower(P<0.05) after treatment,and the level of serum testosterone and PSA in the observation group were significantly higher(P<0.05) than the controlled group.The total clinical effective rate was 87.8%(36/41) in the observation group and 66.7%(28/42) in the controlled group,which was significantly higher in the observation group than in the controlled group(P<0.05).Conclusion:The effect of Bushen Yinian Recipe on these patinets is notable,the recipe can not only improve the clinical symptoms,regulate the level of serum testosterone and PSA,but also improve the cognitive function and relieve the bad mood of anxiety and depression.
