中西医结合加速康复外科理念在腹腔镜胃癌根治术围手术期的应用

Study on application of concept of integrated traditional Chinese and Western medicine of enhanced recovery after surgery(ERAS)in perioperative period of laparoscopic radical resection of gastric cancer

目的探讨中西医结合加速康复外科理念(ERAS)应用于腹腔镜胃癌根治术围术期的临床价值。方法前瞻性纳入2016-02—2019-12间河南中医药大学第一附属医院普外二区行腹腔镜胃癌根治术的患者,根据围术期处理方法分为中西医结合ERAS组(观察组)和ERAS组。比较2组患者的基线资料、围术期相关指标和住院费用。结果共纳入60例患者,每组30例。2组患者的基线资料及术后并发症发生率差异均无统计学意义(P>0.05)。观察组患者术后肠鸣音恢复时间、肛门首次排气排便时间、胃管拔除时间、进食时间、腹腔引流管拔除时间、住院时间,以及住院费用均显著优于ERAS组,差异有统计学意义(P<0.05)。结论中西医结合ERAS应用于腹腔镜胃癌根治术患者是安全、有效的,可加速患者术后的康复进程。

Objective To investigate the clinical effect of the concept of integrated traditional Chinese and Western medicine of enhanced recovery after surgery(ERAS)in the perioperative period of laparoscopic radical resection of gastric cancer.Methods Patients undergoing laparoscopic radical gastrectomy from February 2016 to December 2019 in the second ward of General surgery,the first affiliated Hospital of Henan University of TCM were prospectively enrolled.According to the perioperative management methods,they were divided into the integrated traditional Chinese and Western medicine of ERAS group(the observation group)and the ERAS group.The baseline data,perioperative related indicators and hospitalization expenses were compared between the two groups.Results A total of 60 patients were included,30 cases in each group.There was no significant difference in baseline data and postoperative complications between the two groups(P>0.05).The recovery time of bowel sounds,the first anal exhaust and defecation time,the time of gastric tube extraction,eating time,abdominal drainage tube extraction time,hospitalization time and hospitalization expenses of the observation group were significantly better than those of the ERAS group,and the difference was statistically significant(P<0.05).Conclusion It is safe and effective to apply the concept of integrated traditional Chinese and Western medicine of enhanced recovery after surgery in patients with laparoscopic radical gastrectomy,which can accelerate the recovery process of patients.
