术中洛铂腹腔灌洗化疗对结直肠癌患者肠道功能、肝肾功能及术后并发症的影响

Effect of Intraoperative Intraperitoneal Perfusion Chemotherapy with Lobaplatin on Intestinal Function,Liver and Kidney Function and Postoperative Complications in Patients with Colorectal Cancer

目的探究术中洛铂腹腔灌洗化疗对结直肠癌患者肠道功能、肝肾功能及术后并发症的影响。方法收集2016年7月—2018年7月接受结直肠癌根治术的患者87例为研究对象,其中41例仅接受FLOFOX(氟尿嘧啶+奥沙利铂+亚叶酸钙)方案化疗作为对照组,另46例接受洛铂腹腔灌洗化疗联合FLOFOX方案化疗作为洛铂组。比较2组术后肠道功能恢复情况、实验室检测指标、术后并发症、不良反应及复发转移情况。结果2组肠道排气时间、排便时间、拔管时间、术后住院时间、各时间点实验室检测指标、术后并发症发生率、不良反应发生率、远处转移率及术后6个月时复发率比较差异均无统计学意义(P>0.05)。洛铂组术后引流量显著高于对照组,术后12个月时复发率明显低于对照组(P<0.05)。结论结直肠癌患者术中行洛铂腹腔灌洗化疗可有效预防术后复发,但并不会加重不良反应和术后并发症,具有较好的耐受性和安全性。

Objective To investigate the effect of intraoperative intraperitoneal perfusion chemotherapy with Lobaplatin on intestinal function,liver and kidney function and postoperative complications in patients with colorectal cancer(CRC).Methods A total of 87 patients undergoing radical resection of CRC from July 2016 to July 2018 were collected as research subjects,41 of whom received FLOFOX(Fluorouracil+Oxaliplatin+Calcium leucovorin)chemotherapy alone as the control group,and the other 46 patients received intraperitoneal perfusion chemotherapy with Lobaplatin combined with FLOFOX chemotherapy as the Lobaplatin group.Postoperative intestinal function recovery,laboratory test indexes,postoperative complications,adverse reactions,recurrence and metastasis were compared between the two groups.Results There were no significant differences in intestinal exhaust time,defecation time,extubation time,length of postoperative hospitalization,laboratory test indexes at each time point,postoperative complication rate,adverse reaction rate,distant metastasis rate and recurrence rate at 6 months after operation between the two groups(P>0.05).Postoperative drainage volume was significantly higher in the Lobaplatin group than in the control group,and the recurrence rate at 12 months after operation was significantly lower in the Lobaplatin group than in the control group(P<0.05).Conclusion Intraperitoneal perfusion with Lobaplatin can effectively prevent postoperative recurrence in patients with CRC,but it does not aggravate adverse reactions and postoperative complications,and has good tolerance and safety.
