中药煎服联合穴位贴敷佐治支气管扩张伴咯血临床观察

Traditional Chinese Medicine Decoction Combined with Acupoint Application for Bronchiectasis with Hemoptysis

目的探讨中药煎服联合穴位贴敷佐治支气管扩张伴咯血的临床疗效。方法选取医院2017年6月至2019年6月收治的支气管扩张伴咯血患者120例,随机分为A组、B组、C组、D组,各30例。4组患者均予青霉素、安溴索、垂体后叶素常规治疗,B组患者加用中药方剂口服,C组患者加用中药穴位贴敷,D组患者加用中药方剂口服及中药穴位贴敷。4组均持续治疗7 d。结果总有效率方面,B组、C组、D组均显著高于A组,D组显著高于B组和C组(P<0.05);退热、咳嗽、湿罗音、咳血等症状体征消退时间方面,B组、C组、D组均显著短于A组,D组显著短于B组和C组(P<0.05);累计咳血量方面,B组、C组、D组显著少于A组,D组显著少于B组和C组(P<0.05);第1秒用力呼气容积(FEV1)占预计值百分比、用力肺活量(FVC)、最大呼气流速(PEF)方面,B组、C组、D组显著高于A组,D组显著高于B组和C组(P<0.05);A组、B组、C组、D组不良反应发生率相当(6.67%比10.00%比13.33%比16.67%,P>0.05)。结论中药煎服联合穴位贴敷佐治支气管扩张伴咯血,能加速症状体征缓解,减少咯血量,改善肺功能。

Objective To study the clinical efficacy of traditional Chinese medicine decoction combined with acupoint application for bronchiectasis with hemoptysis.Methods A total of 120 patients with bronchiectasis and hemoptysis treated in the hospital from June2017 to June 2019 were selected and randomly divided into groups A,B,C and D,30 cases in each group.All 4 groups were treated with penicillin,ambroxol,and pituitrin.Group B was added with traditional Chinese medicine decoction orally,group C was added with acupoint application,and group D was added with traditional Chinese medicine decoction combined with acupoint application.All groups received 7 d of continuous treatment.Results In total effective rate,groups B,C,D were significantly higher than group A,and group D was significantly higher than groups B and C(P<0.05).In the time to resolve symptoms and signs such as fever,cough,sputum,moist rales,hemoptysis and hemoptysis,groups B,C,D were significantly shorter than group A,and group D was significantly shorter than groups B and C(P<0.05).In cumulative hemoptysis,groups B,C,D were significantly less than group A,and group D was significantly less than groups B group C(P<0.05).In forced expiratory volume(FEV1),forced vital capacity(FVC),maximum expiratory flow(PEF),groups B,C,D were significantly higher than group A,and group D was significantly higher than groups B and C(P<0.05);The incidences of adverse reactions of groups A,B,C,D were equivalent(6.67%vs.10.00%vs.13.33%vs.16.67%,P>0.05).Conclusion Traditional Chinese medicine decoction combined with acupoint application for bronchiectasis with hemoptysis can accelerate the remission of symptoms,reduce the hemoptysis volume,and improve the pulmonary function index of patients.
