参与式扶贫模式在县医院护理管理中的实践效果

Practical effects of participatory poverty alleviation pattern in nursing management of county hospitals

目的探讨参与式扶贫模式在县医院护理管理中的实践效果。方法三级医院成立“阜平医院工作组”,派驻护理管理者2名,通过与县医院护理管理者座谈,到临床科室观察护理现状,梳理护理工作中存在问题,针对问题制定护理帮扶方案,应用参与式扶贫模式落实该方案。结果2017至2018年增加护理人员28名,有效缓解了人力匮乏;培养省级专科护士3名,外派护士长和护理骨干到三级医院免费进修25人次;修订临床常见护理技术操作标准20项,累计完成护士长和护士培训1150余人次;修订护理管理制度及关键流程50余项,制定了护理质量管理方案,明确了各级人员职责;用药差错发生率由2016年0.39%下降至2019年0.01%,差异有统计学意义(P<0.01),压力性损伤现患率由培训前2.69%下降为0.39%,差异有统计学意义(P<0.05);县医院患者外转率从2016年的11.20%降至2019年的6.83%,差异有统计学意义(P<0.05)。结论参与式扶贫模式在县医院护理管理中的实践,不仅提高了护理人员工作积极性,还有效地降低不良事件发生,提高了县医院护理水平。

Objective To investigate the practical effects of participatory poverty alleviation pattern in nursing management of county hospitals.Methods The“Fuping Hospital Working Group”was established in the tertiary hospital.and 2 nursing managers were assigned to the Fuping Hospital.By having an informal discussion with nursing managers of the county hospital and viewing nursing status in clinical departments,the nursing managers formulated nursing assistance plan according to the problems and implemented it by using participatory poverty alleviation mode.Results A increase of 28 nurses from 2017 to 2018 effectively alleviated manpower shortage,and 3 provincial specialist nurses were cultivated,and a total 25 head nurses and nursing backbone members were sent to the tertiary hospital for free training.Moreover 20 clinical nursing technical operating standards were revised,and a total of 1,150 head nurses and nurses had been trained,and more than 50 items of nursing management regulation and key processes were revised,and a care quality management program was set up,and the responsibilities of personnel at all levels were clarified.After that the incidence rate of medication errors was decreased from 0.39%in 2016 to 0.01%in 2019(P<0.01),and the incidence rate of stress injuries was decreased from 2.69%before training to 0.39%after training(P<0.05).In addition the outward transferring rate of county hospital was decreased from 11.20%in 2016 to 6.83%in 2019(P<0.05).Conclusion The application of participatory poverty alleviation pattern in county hospital can not only enhance the enthusiasm of nursing staff,but also effectively reduce the incidence of adverse events,so as to improve the nursing levels in the county hospital.
