小切口开颅显微技术切除桥小脑角区肿瘤治疗继发性三叉神经痛的疗效

Effect of Small Incision Craniotomy Microsurgery to Remove Tumors in the Cerebellopontine Angle Area in the Treatment of Secondary Trigeminal Neuralgia

摘要 目的观察与评价小切口开颅显微技术切除桥小脑角区肿瘤治疗继发性三叉神经痛的疗效。方法对该院于2016年2月—2018年11月收治的桥小脑角区肿瘤继发性三叉神经痛患者,从中选择18例作为该次研究的对象。所有患者均接受了小切口开颅显微技术治疗,观察18例患者手术治疗术后住院时间、术后并发症发生率、术后随访复发情况、术后House-Brackmann面神经功能分级情况。结果所有患者经过小切口开颅显微技术切除桥小脑角区肿瘤治疗后,三叉神经痛症状消失。15例患者术中肿瘤全部切除,其余病例全切囊壁。术后随访半年发现,House-Brackmann面神经功能分级Ⅰ级11例,Ⅱ级4例,Ⅲ级1例,Ⅴ级1例,神经功能保留率为88.89%。随访发现术后1例复发,复发率为5.56%。结论对桥小脑角区肿瘤治疗继发性三叉神经痛患者采用小切口开颅显微技术治疗可以消除患者三叉神经痛症状,且术后面部神经功能保留率较高、复发率低,疗效良好。 Objective To observe and evaluate the effect of small incision craniotomy microsurgery to remove tumors in the cerebellopontine angle area in the treatment of secondary trigeminal neuralgia.Methods Eighteen patients with secondary trigeminal neuralgia from pontine cerebellar corner tumors treated in the hospital from February 2016 to November 2018 were selected as subjects of this study.All patients underwent small incision craniotomy microscopy.Observation of hospitalization time,incidence of postoperative complications,postoperative follow-up recurrence,and House-Brackmann facial nerve function grading of 18 patients after surgical treatment were observed.Results After all patients underwent small incision craniotomy microsurgery to remove tumors in the cerebellopontine angle area,the symptoms of trigeminal neuralgia disappeared.Fifteen patients had all tumors removed during the operation,and the remaining cases had their cyst wall removed.After six months of follow-up,it was found that House-Brackmann's facial nerve function was graded in 11 cases in gradeⅠ,4 cases in gradeⅡ,1 case in gradeⅢ,and 1 case in gradeⅤ.The nerve function retention rate was 88.89%.Follow-up found that 1 case relapsed after operation,the recurrence rate was 5.56%.Conclusion The treatment of patients with secondary trigeminal neuralgia in cerebellopontine angle tumors with small incision craniotomy can eliminate the symptoms of trigeminal neuralgia,and the facial nerve function retention rate is high,the recurrence rate is low,and the effect is good.
