摘要
目的探究高频彩超在基层乳腺癌患者筛查中的作用。方法选取2009年1月—2018年1月对该市基层社区的28544名妇女进行乳腺癌筛查,进行高频彩超及超声造影检测;统计乳腺癌患者的病理检查结果及分期;血流丰富程度及类型;统计乳腺癌患者超声及超声造影表现特征。结果28544名妇女中检测到乳腺组织异常的并手术切除患者41例,确诊为乳腺癌的患者30例。血流丰富程度:Ⅰ级5例(16.67%),Ⅱ级18例(60.00%),Ⅲ级7例(23.33%);血流阻力RI≥0.7的患者11例(36.67%),极低回声患者15例(50.00%),低~极低回声的患者9例(30.00%),高回声患者6例(20.00%),以低回声及极低回声患者数量较多,且多表现为蟹足样、边缘成角,有部分患者出现细小钙化斑,并有2例患者出现乳头红色分泌物。肿块大小低于10 mm的患者4例,10~20 mm的患者19例,大于20 mm的患者7例。结论高频彩超在基层乳腺癌患者的筛查中具有重要意义,乳腺癌的检出率高,能够为临床诊断治疗提供一定的临床依据。
Objective To explore the role of high-frequency color Doppler ultrasound in the screening of primary breast cancer patients.Methods From January 2009 to January 2018,28544 women in the grassroots community of the city were screened for breast cancer,high-frequency color Doppler ultrasound and ultrasound contrast examination were performed;the pathological examination results and stages of breast cancer patients were counted;blood flow richness level and type;statistical characteristics of breast cancer patients with ultrasound and ultrasound contrast.Results Among 28544 women,41 patients with abnormal breast tissue were detected and surgically resected,and 30 patients were diagnosed with breast cancer.Blood flow abundance:Ⅰlevel 5 cases(16.67%),gradeⅡ18 cases(60.00%),Ⅲlevel 7 cases(23.33%);Blood flow resistance RI acuity 0.7 of 11 cases(36.67%)patients,15 patients with extremely low echo(50.00%),low–extremely low echo of 9 cases(30.00%)patients,6 patients with high echo(20.00%),with a low echo and extremely low echo patient number is more,and more performance for keloid,edge angled,there were some patients appear tiny calcified plaque,and 2 patients appeared red nipple discharge.There were 4 patients with tumor size less than 10 mm,19 patients with size between 10 and 20 mm,and 7 patients with size greater than 20 mm.Conclusion High-frequency color Doppler ultrasound is of great significance in the screening of primary breast cancer patients.The high detection rate of breast cancer can provide a certain clinical basis for clinical diagnosis and treatment.
