未足月胎膜早破孕妇剩余羊水量与母儿结局关系探讨

Study on the Relationship between Remaining Amniotic Fluid of Pregnant Women with Premature Rupture of Membranes and Maternal and Child Outcomes

目的探讨孕妇在未足月时发生胎膜早破后体内剩余羊水量与母儿妊娠结局的关系。方法选取2017年10月—2019年10月于该院就诊的PPROM孕妇74例,根据孕妇胎膜破裂后的AFI将其分为正常组、偏少组和过少组,观察3组孕妇破膜时间、WBC、胎盘早剥、剖宫产等情况的发生情况,观察胎儿出生后NRDS、呼吸衰竭、HIE等不良事件的发生情况。结果过少组潜伏期(2.7±1.3)d,低于偏少组(5.3±2.7)d,低于正常组(22.7±3.4)d,组间比较(F=10.423,P<0.05);过少组剖宫产率70.6%,偏少组剖宫产率66.7%,均高于正常组39.4%,组间比较(χ^2=8.524,P<0.05)。结论孕妇发生PPROM后的剩余羊水量过少会导致孕妇潜伏期缩短,孕妇剖宫产、胎儿窘迫等不良事件的发生率增高。

Objective To explore the relationship between the amount of amniotic fluid remaining in the body and the pregnancy outcome of the mother and child after premature rupture of membranes in pregnant women.Methods From October 2017 to October 2019,74 cases of PPROM pregnant women who were treated in the hospital were divided into normal group,partial group and too few group according to the AFI after rupture of fetal membranes.Membrane time,WBC,placental abruption,cesarean section and other conditions,and observe the occurrence of adverse events such as NRDS,respiratory failure,HIE after birth.Results The incubation period of the too little group was(2.7±1.3)d,which was lower than that of the lesser group(5.3±2.7)d and lower than that of the normal group(22.7±3.4)d.Comparison between groups(F=10.423,P<0.05);The cesarean section rate in the less group was 70.6%,and the cesarean section rate in the lesser group was 66.7%,which was higher than that in the normal group,39.4%(χ^2=8.524,P<0.05).Conclusion Too little residual amniotic fluid after PPROM in pregnant women will shorten the latency period of pregnant women,and increase the incidence of adverse events such as cesarean section and fetal distress.
