针灸联合散瘀止痛汤治疗膝骨性关节炎疗效影响

Effect of acupuncture combined with Sanyu Zhitong Decoction on knee osteoarthritis

目的探讨针灸联合散瘀止痛汤对KOA膝关节功能影响,及对关节滑液中白介素1β和血管内皮生长因子(VEGF)水平影响。方法选择120例KOA患者,按采用随机数字法原则将患者随机分为观察组1、观察组2和观察组3,每组各40例;观察组1给予散瘀止痛汤治疗,观察组2给予针灸治疗,观察组3给予针灸联合散瘀止痛汤治疗;检测3组治疗前后Lysholm膝关节评分及关节滑液中白介素1β和VEGF浓度变化。结果3组治疗后第2周和第4周Lysholm膝关节评分比较,差异有统计学意义(P<0.01);治疗第4周3组关节滑液中白介素1β和VEGF浓度差异有统计学意义(P<0.01);关节滑液中白介素1β和VEGF浓度观察组3较观察组1和观察组2显著降低(P<0.01)。结论针灸联合散瘀止痛汤能显著改善膝关节功能,显著降低关节滑液中白介素1β和VEGF水平。

Objective To investigate the effects of acupuncture combined with Sanyu Zhitong Decoction on the function of KOA knee joint and the levels of interleukin-1βand VEGF in synovial fluid.Methods A total of 120 patients with knee osteoarthritis were selected and were randomly divided into observation group 1,observation group 2 and observation group 3,adopting a random number method,with 40 cases in each group;the observation group 1 was treated with Sanyu Zhitong Decoction,and the observation group 2 was given acupuncture,and the observation group 3 was given acupuncture combined with Sanyu Zhitong Decoction.The Lysholm knee score,and the changes of the of interleukin 1 beta and vascular endothelial growth factor levels in the synovial fluid were detected before and after treatment among the 3 groups.Results Lysholm knee score was significantly different between the 3 groups at week 2 and 4 after treatment,with the differences statistically significant(P<0.01).There was statistical significance on the differences in the concentrations of interleukin 1βand VEGF in the synovial fluid of the three groups at week 4(P<0.01);the concentrations of interleukin 1βand VEGF in synovial fluid were significantly lower in group 3 than that in group 1 and group 2(P<0.01).Conclusion Acupuncture combined with Sanyu Zhitong Decoction can significantly improve knee function and decrease interleukin 1βand VEGF levels in synovial fluid.
