新型冠状病毒疫情期间病情稳定精神分裂症患者抑郁焦虑水平及相关因素分析

Mental health status and its influencing factors among patients in the hospital with stable schizophrenia during the outbreak of Coronavirus disease 2019

目的比较新型冠状病毒疫情前和疫情期间病情稳定的住院精神分裂症患者抑郁及焦虑水平的差异,并对抑郁,焦虑水平与影响因素进行相关分析,为疫情期间精神科医生诊疗提供帮助。方法选择疫情前和疫情期间沈阳市精神卫生中心住院病情稳定的精神分裂症患者110例(男51例,女59例),分别调阅疫情前HAMA,HAMD得分,并评定疫情期间HAMA,HAMD得分,采用自制量表调查患者住院期间接触疫情信息时长,医生宣讲疫情次数以及家属来电次数,对结果进行比较并做相关性分析。结果与疫情前相比疫情期间患者HAMA得分显著升高(P<0.01);HAMD得分无显著差异(P>0.05);HAMA得分与患者接触疫情信息时长(r=0.70,P<0.01)呈正相关;与医生宣讲疫情信息次数(r=-0.476,P<0.01),家属来电次数(r=-0.619,P<0.01)呈负相关。结论疫情期间,精神科医师需关注病情稳定的住院精神分裂症患者的焦虑水平,减少患者接触疫情信息时长,增加对患者新冠病毒知识的宣讲及家属与患者电话沟通次数。

Objective Compare the differences of depression and anxiety levels in the patients with stable schizophrenia between the daily and outbreak of COVID-19,and analyze the differences of depression and anxiety among patients caused by different factors. Methods A total of 110 schizophrenia patients with stable conditions were selected from Shenyang mental health center before and during the COVID-19 epidemic,to review the score of HAMA and HAMD before the outbreak and investigate HAMA,HAMD scores during the epidemic period,a self-designed scale was used to investigate the duration of exposure to epidemic information,the number of times doctors reported the epidemic,and the number of calls from family members,Compare the results and do correlation analysis. Results During the epidemic period HAMA score of patients was significantly higher than that before the epidemic(P<0.01). There was no significant difference between the HAMD score before and during the epidemic(P> 0.05). HAMA score was positively correlated with the duration of exposure to epidemic information(r=0.70,P<0.01). There was a negative correlation with the number of times doctors reported the epidemic(r=-0.476,P<0.01)and the number of calls from family members(r=-0.619,P<0.01). Conclusion During the epidemic,the psychiatrist should pay attention to the anxiety level of hospitalized schizophrenic patients with stable condition,reduce the time of the patients’ exposure to the epidemic information,increase the publicity of the patient’s novel coronavirus knowledge and the number of telephone communication between the family and the patient.
