摘要
目的探讨血清半乳糖凝集素-3(Gal-3)水平联合CHA2DS2-VASc评分对非瓣膜性房颤患者发生缺血性脑卒中的预测价值。方法选取2017年8月-2018年8月在本院接受治疗的非瓣膜性房颤患者226例,所有患者均进行为期1年的随访,根据随访将发生缺血性脑卒中的患者纳入到脑卒中组(32例),将未发生缺血性脑卒中的患者纳入到无脑卒中组(194例),收集患者的一般资料及常规指标水平,采用酶联免疫吸附试验检测血清Gal-3的水平,记录所有患者的CHA2DS2-VASc评分。结果脑卒中组的收缩压、左心房内径(LAD)、Gal-3水平、CHA2DS2-VASc评分均高于无脑卒中组(P<0.05);LAD、Gal-3水平、CHA2DS2-VASc评分过高均是非瓣膜性房颤患者发生缺血性脑卒中的独立危险因素(P<0.05);非瓣膜性房颤患者的血清Gal-3水平与空腹血糖、LAD、CHA2DS2-VASc评分均呈正相关(P<0.05);ROC分析显示,Gal-3水平与CHA2DS2-VASc评分对非瓣膜性房颤患者发生缺血性脑卒中的预测价值均较高,曲线下面积分别为0.708和0.797,而二者联合分析可使得预测价值进一步提升。结论血清Galectin-3水平联合CHA2DS2-VASc评分对非瓣膜性房颤患者发生缺血性脑卒中的预测价值较高。
Objective To explore the predictive value of serum galectin-3(Gal-3) combined with cha2 ds2-vasc score for ischemic cerebral stroke in patients with non-valvular atrial fibrillation. Methods 226 patients with non-valvular atrial fibrillation who were treated in our hospital from August 2017 to August 2018 were selected. All patients were followed up for one year. According to the follow-up data, the patients with ischemic cerebral stroke were included in the cerebral stroke group(32 cases), and the patients without ischemic cerebral stroke were included in the non cerebral stroke group(194 cases). The general data and routine parameter levels of all patients were collected, the level of serum Gal-3 was detected by enzyme-linked immunosorbent assay, and the cha2 ds2-vasc score of all patients were recorded. Results Systolic pressure, left atrial diameter(LAD), Gal-3 level and cha2 ds2-vasc score of cerebral stroke group were higher than those of non cerebral stroke group(P<0.05). The too high LAD, Gal-3 level and cha2 ds2-vasc score were all independent risk factors for ischemic stroke in patients with non-valvular atrial fibrillation(P<0.05). The serum Gal-3 level was positively correlated with fasting blood glucose, LAD and cha2 ds2-vasc score(P<0.05). ROC analysis showed that Gal-3 level and cha2 ds2-vasc score had a higher predictive value for ischemic cerebral stroke in patients with non-valvular atrial fibrillation. The area under the curve was 0.708 and 0.797 respectively. The combined analysis of the two could further improved the predictive value. Conclusion The combination of serum galectin-3 level and cha2 ds2-vasc score had a high predictive value for ischemic cerebral stroke in patients with non-valvular atrial fibrillation.
