sdLDL-C、IMA、25-OH-VD与急性缺血性脑卒中的相关性研究

Study on correlation between sdLDL-C,IMA,25-OH-VD and acute ischemic st-roke

目的:探讨血清小而密低密度脂蛋白(sdLDL-C)、缺血修饰蛋白(IMA)、25-羟基维生素D(25-OH-VD)与急性缺血性脑卒中(AIS)类型、病情及预后的关系。方法:检测150例AIS患者(AIS组)和150例志愿者(对照组)血清sdLDL-C、IMA、25-OH-VD水平。观察不同AIS分型(OCSP分型)、病情(NIHSS评分、梗死面积)、预后患者血清sdLDL-C、IMA、25-OH-VD水平差异。分析血清sdLDL-C、IMA、25-OH-VD水平与AIS预后关系及对AIS预后的预测价值。结果:AIS组患者血清sdLDL-C、IMA水平高于对照组(P<0.05),25-OH-VD水平低于对照组(P<0.05),血清sdLDL-C、IMA水平随神经损伤程度加重和梗死面积增加而升高(P<0.05),血清25-OH-VD水平降低(P<0.05)。好转组血清sdLDL-C、IMA水平低于无好转组(P<0.05),25-OH-VD水平高于无好转组(P<0.05),血清sdLDL-C、IMA、25-OH-VD水平为AIS预后独立预测因子,AUC分别为0.834、0.776、0.725。结论:血清sdLDL-C、IMA、25-OH-VD与AIS患者病情进展和预后密切相关,可作为评估AIS患者病情、预后的辅助生物学指标。

Objective:To investigate correlation between serum small and dense low density lipoprotein(sdLDL-C),ischemic modified protein(IMA),25-hydroxyvitamin D3(25-OH-VD)and type,condition and prognosis of acute ischemic stroke(AIS).Methods:Serum level of sdLDL-C,IMA and 25-OH-VD were measured in 150 patients with AIS(AIS group)and 150 volunteers(control group).Observed difference of serum levels of sdLDL-C,IMA and 25-OH-VD among different AIS types(OCSP),condition(NIHSS scores,infarct size)and prognosis.Analyzed relationship between serum level of sdLDL-C,IMA,25-OH-VD and prognosis of AIS and its predictive value.Results:Levels of serum sdLDL-C and IMA in AIS group were higher than those in control group(P<0.05),25-OH-VD was lower than that in control group(P<0.05).Levels of serum sdLDL-C and IMA were increased with severity of nerve injury and increase of infarction area(P<0.05),while level of serum 25-OH-VD was decreased(P<0.05).Serum levels of sdLDL-C and IMA in improved group were lower than those in non-improved group(P<0.05),and 25-OH-VD was higher than those in non-improved group(P<0.05).Serum levels of sdLDL-C,IMA and 25-OH-VD were independent predictors of AIS prognosis,whose AUC were 0.834,0.776 and 0.725,respectively.Conclusion:Serum sdLDL-C,IMA and 25-OH-VD levels are closely related to progression and prognosis of AIS patients,and can be used as auxiliary biological indicators to evaluate condition and prognosis of AIS patients.
