血清降脂素及维生素D与2型糖尿病患者胰岛β细胞功能及胰岛素敏感性的相关性

Correlations of serum adipsin and Vit D levels with islet beta cell function and insulin sensitivity in patients with type 2 diabetes mellitus

目的探讨血清降脂素、维生素D(Vit D)水平与2型糖尿病(T2DM)患者胰岛β细胞功能及胰岛素敏感性的相关性。方法检测60例T2DM患者(T2DM组)、60例糖耐量减低(IGT)患者(IGT组)、60名体检健康者(正常对照组)的空腹血糖(FPG)、空腹胰岛素(FINS)、降脂素、Vit D水平,同时计算急性胰岛素反应(AIR)、稳态模型-β细胞功能指数(HOMA-β)、胰岛素敏感指数(ISI)。采用多因素Logistic回归分析评估血清降脂素、Vit D的影响因素。采用Pearson相关分析评估各项目之间的相关性。结果T2DM组血清降脂素、Vit D水平及HOMA-β、ISI、AIR均低于IGT组和正常对照组(P<0.05),FPG、FINS水平均高于IGT组和正常对照组(P<0.05)。3组之间体质量指数(BMI)及腰臀比差异均有统计学意义(P<0.05),性别、年龄差异均无统计学意义(P>0.05)。HOMA-β、ISI、AIR是影响血清降脂素、Vit D水平的因素(P<0.05)。T2DM患者血清降脂素、Vit D与HOMA-β、ISI均呈正相关(P<0.05),血清降脂素与Vit D呈正相关(P<0.05)。结论血清降脂素、Vit D水平与T2DM患者胰岛β细胞功能及胰岛素敏感性有关。

Objective To investigate the correlations of serum adipsin and vitamin D(Vit D)with islet beta cell function and insulin sensitivity in patients with type 2 diabetes mellitus(T2DM).Methods A total of 60 patients with T2DM and 60 patients with impaired glucose tolerance(IGT)were enrolled,and 60 healthy subjects were enrolled as control group.Serum adipsin,fasting plasma glucose(FPG),fasting insulin(FINS)and Vit D were determined.The acute insulin response(AIR),the homeostasis model assessment beta(HOMA-β)and insulin sensitivity index(ISI)were calculated.Multivariate Logistics regression analysis was used to study the influencing factors of serum adipsin and Vit D,and Pearson correlation analysis was used to study the correlations of serum adipsin and Vit D with AIR,HOMA-βand ISI.Results The levels of serum adipsin and Vit D in T2DM group were lower than those in IGT group and control group,while the levels of FPG and FINS were higher than those in IGT group and control group(P<0.05).Body mass index(BMI)and waist-to-hip ratio had statistical significance among the 3 groups(P<0.05),and sex and age had no statistical significance(P<0.05).HOMA-β,ISI and AIR were the risk factors for serum adipsin and Vit D levels(P<0.05).Serum adipsin and Vit D in T2DM patients were positively correlated with HOMA-βand ISI(P<0.05),while serum adipsin and Vit D were positively correlated(P<0.05).Conclusions Serum adipsin and Vit D levels are correlated with islet beta cell function and insulin sensitivity in patients with T2DM.
