截石位放射治疗腿部固定托架的研制及临床应用

摘要 目的:研制截石位放射治疗腿部固定托架,改良会阴部浅表肿瘤放射治疗膀胱截石体位固定技术。方法:设计并制作截石位放射治疗腿部固定托架。选取23例行调强放射治疗(IMRT)的会阴部浅表肿瘤患者,均采取膀胱截石体位进行固定,按照随机数表法将其分为对照组(11例)和观察组(12例),对照组采用膝部垫+热塑膜固定方式,观察组采用截石位放射治疗腿部固定托架+热塑膜固定方式,分析比较两组患者的摆位误差。结果:通过对两组患者摆位误差的比较,观察组采用截石位放射治疗腿部固定托架进行体位固定的患者,在Y轴(前后)和Z轴(头足)方向的摆位误差优于对照组,其差异有统计学意义(t=3.826,t=-1.862;P<0.05)。结论:截石位放射治疗腿部固定托架的使用提高了会阴部浅表肿瘤放射治疗的体位固定技术,使会阴部充分暴露,利于放射治疗计划的优化设计。 Objective: To develop fixed bracket on leg of lithotomy position in radiotherapy and improve the fixation technique of bladder lithotomy position for radiotherapy on superficial tumor of perineum. Methods: All 23 patients with superficial tumor of perineum who underwent intensity modulated radiation therapy(IMRT) were selected and were fixed by bladder lithotomy position, and they were divided into control group(11 cases) and observation group(12 cases) by random number table. And the control group was fixed with knee pad and thermoplastic film, and the observation group was fixed with fixed bracket on leg of lithotomy position in radiotherapy and thermoplastic film. The positioning errors of two groups were analyzed and compared. Results: Through the comparison of the positioning error between the two groups, the positioning errors at Y-axis(front and back) and Z-axis(head and foot) of observation group were significantly better than those of control group(t=3.826, t=-1.862, P<0.05). Conclusion: The use of this bracket improves the technique of position fixation of radiotherapy on the superficial tumor of perineum. And the fully exposed perineum is beneficial to the optimization design of the radiotherapy plan.
