摘要 目的:研究制定心脏除颤监护仪质量控制周期和靶值标准差(SD)标准,构建除颤设备系统化质量安全管理体系。方法:选取5家医院在用的除颤监护仪,对故障设备采用鱼骨图剖析,绘制柏拉图分析主因,从人员、设备及配置方面进行质量安全管理。通过采集并整合完善除颤监护仪质量控制周期及SD靶值能量等数据,分析成因,动态跟踪除颤监护仪故障及不良事件发生频次。结果:心脏除颤监护仪质量控制周期和靶值SD标准的制定,从质量控制层面解决了现阶段医疗机构无明确除颤质量控制周期及靶值SD规范的标准问题,避免因设备质量控制不完善致使抢救失败等不良事件发生。结论:构建完善的除颤监护仪质量控制体系需各部门通力合作,建立完整的制度和监督办法,质量控制周期和靶值SD标准能够显著降低心脏除颤监护仪故障及不良事件,提高除颤成功率,为患者提供更好的医疗服务。 Objective: To study and formulate the quality control cycle and standard of standard deviation(SD) of target value of cardiac defibrillation monitor, and explore and practice the systematic quality and safety management system of defibrillation equipment. Methods: The using defibrillator monitors of 5 hospitals were selected, and the fishbone diagram was used to analyze the equipment with failure, and Plato chart was drawn to analyze the main cause of failure. And the quality safety management was carried out from three aspects included personnel, equipment and configuration. The reasons were analyzed, and the frequencies of faults and adverse events of defibrillation monitors were dynamically traced through collected, integrated and improved the quality control cycle, the power of SD target value and others of defibrillation monitors. Results: The formulation of quality control cycle and SD standard of target value of cardiac defibrillation resolved a problem that there was no explicit standard of quality control cycle and SD criterion of target value of defibrillation in medical institutions at present from the aspect of quality control, and avoided a series of adverse event such rescue fault and so on caused by imperfection of quality control of equipment. Conclusion: The impeccable quality control systems of defibrillation monitor need cooperation with multi-aspect to establish comprehensive regime and supervision method. The quality control cycle and SD standard of target value can significantly decrease the fault and adverse event of cardiac defibrillation monitor, and increase successful rate of defibrillation, and provide better medical service for patients.
