某市基层医疗机构医疗纠纷现状分析与对策研究

Analysis and Research on the Status Quo and Countermeasures of Medical Disputes in the Grassroots Medical Institutions of a City

目的:分析某市基层医疗卫生机构213例医疗纠纷案例,总结该地区基层医疗机构医疗纠纷特点,为预防、减少医疗纠纷提出针对性措施。方法:用SPSS13.0建立数据库,运用文献复习法和描述性分析的方法,分析不同年份不同类别医院发生纠纷例数、纠纷发生主要原因、纠纷发生科室、处理方法以及医疗事故情况等。结果:该市四年发生的医疗纠纷例数逐年增多以县级医院为主;在引起纠纷因素中,医疗技术水平不高占首位,其次是服务态度不良,责任心不强;外科、骨科、妇产科是医疗纠纷高发的科室;医疗纠纷发生后采取的解决方式以卫生行政部门调节方式为主;医疗事故无明显增加。结论:加强医院管理,提高医疗水平,增进服务意识,促进医患沟通,关注重点科室,健全处理机制是有效减少医疗纠纷的措施。

Objective:To analyze 213 cases of medical disputes in the grassroots medical institutions of a city,summarize the characteristics of medical disputes in the grassroots medical institutions of the area,and propose targeted measures to prevent and reduce medical disputes.Method:To use SPSS13.0 to establish a database,and use literature review and descriptive analysis methods to analyze the number of disputes in different types of hospitals in different years,the main reasons for the disputes,the departments where the disputes occurred,the handling methods,and the situation of medical accidents.Results:The number of medical disputes that occurred in the city over the past four years has increased year by year,mainly in county-level hospitals;among the factors that caused disputes,the low level of medical technology took the first place,followed by poor service attitudes and weak sense of responsibility;department of surgery,department of orthopedics,and department of obstetrics and gynecology are departments with a high incidence of medical disputes;after the occurrence of medical disputes,the handling methods are mainly based on the mediation of health administrative departments;there is no significant increase in medical accidents.Conclusion:Strengthening hospital management,improving medical standards,enhancing service awareness,promoting doctor-patient communication,focusing on key departments,and improving the handling mechanism,are effective measures to reduce medical disputes.
