探求甲状腺良性肿瘤采用峡部入路改良Miccoli手术和上外侧入路改良Miccoli手术治疗对其疗效的影响

To Investigate the Effect of Modified Isthmic Approach and Modified Upper Lateral Approach For Miccoli on Benign Thyroid Tumors

目的分析峡部入路改良Miccoli手术与上外侧入路改良Miccoli手术在甲状腺良性肿瘤治疗中的应用效果。方法选取2017年1月至2019年5月广东省广州市白云区人民医院收治的甲状腺良性肿瘤患者30例,按照手术方法进行分组,外侧入路改良Miccoli手术治疗的15例患者为对照组,峡部入路改良Miccoli手术治疗的15例患者为观察组,对临床治疗效果进行对比分析。结果手术时间、术后引流量和术中出血量方面,观察组少于对照组,差异有统计学意义(P<0.05);治疗前,两组血清游离甲状腺素(FT4)、血清游离三碘甲状腺原氨酸(FT3)和血清促甲状腺素(TSH)对比,差异无统计学意义(P>0.05);治疗后,观察组高于对照组,差异有统计学意义(P<0.05);手术并发症发生率观察组6.67%低于对照组40.00%,差异有统计学意义(P<0.05)。结论峡部入路改良Miccoli手术在甲状腺良性肿瘤治疗中应用效果比上外侧入路改良Miccoli手术良好,能缩短手术时间、减少术中出血量,也可降低手术并发症的发生率,改善术后甲状腺功能。

Objective To analyze the effect of modified Miccoli operation and superior lateral approach in the treatment of benign thyroid tumors.Methods Totally 30 cases of benign thyroid tumor were received from January 2017 to May 2019 by the people's hospital of Baiyun district,guangzhou city,guangdong province.15 patients with modified Miccoli surgical treatment were divided into control group according to parallel control method.15 patients with modified Miccoli surgical treatment by isthmus approach were compared and analyzed.Results Operation time,postoperative drainage and intraoperative bleeding,the observation group compared with the control group were less,significant difference(P<0.05);FT3、FT4 and TSH,before treatment,the two groups compared,the difference was not significant(P>0.05);after treatment,the observation group was higher than the control group,the difference was obvious(P<0.05);the incidence of complications,the observation group 6.67% compared with the control group 40.00%,the difference is significant(P<0.05).Conclusion The modified Miccoli operation with isthmus approach is more effective than the superior lateral approach in the treatment of benign thyroid tumor.In addition to reducing the operation time,intraoperative bleeding volume,it can reduce the incidence of complications and improve thyroid function.
