3种不同类型的带蒂穿支皮瓣在修复膝下软组织缺损中的应用价值

Application value of three diferent types of pedicled perforator flaps in repairing subknee soft tssue defect

目的:探讨3种不同类型的带蒂穿支皮撕在修复滕下软组织缺损中的应用价值。方法:将2016年2月至2018年2月期间北京北亚骨科医院创伤骨科收治的63例小腿及既关节周團骨折伴软组织缺报患者作为研究对象。对这63例患者分别使用腓动脉带蒂穿支皮善、胫后动脉带蒂穿支皮瓣和胫前动脉带蒂穿支皮舞进行软组织修复。然后,观察这些患者缺损软组织处的修复效果。结果:这63例患者共使用63块带蒂穿支皮瓣,其中有62例患者的皮舞成活,有1例患者移植的胫前动脉穿支皮瓣发生坏死;其切取皮矮的面积为5 cm x 7cm-13 cm x 17 cm,切取皮瓣的.面积多在曼区面积的20%以上;其皮弹愈合的时间为16-21天,皮弹的外观无靠肿、皮色一致、柔软松跑、无皮肤挛缩;其成活的皮弹均未发生再坏死。结论:腓动脉带蒂穿支皮舞、胫后动脉带蒂穿支皮舞和胫前动脉带蒂穿支皮接在修复膝下软组织缺损中均具有较高的应用价值。

Objective:To invesligatle the application value of three dfferet types of pedicled perforalor flaps in repairing subknee soft issue defects.Methods:From February 2016 to February 2018,63 patients with leg and ankle fractures with soft tssue defects were admitted to the Orthopedics department of Beijing North Asia Orthopedics Hopital.The 63 patients received eoft issue repair with peroneal artery pedicled perforator flap,posterior tibial artery pedicled perforator flap and anterior tibial artery pedicled perforator flap repetirely Then,the repair ffr of the defect soft tssue in theae patients wns obsered.Results:A total of 63 pedieled perforntor flap8 were ued in the 63 patients.Among them,the flap8 survived in 62 patients and the transposition of anterir tibial artery flaps waa necrotie in 1 patient.The area of the flapwas5cm x 7cm-13cm x 17 em,and the area of the flap was more than 20%of the area of the receiving area.The healing time of the flap was 16-21 days,and the skin flap had no bloating.uniform skin color,8oft and flabby appearance,and no skin contracture.None of the survived flaps had necroais.Coneluaion:Peroneal artery pedicled perforator flap,posterior tibial artery pedicled perfortor flap and anterior tibial artery pedicled perforalor flap have high application value in repairing subknee soft tssue defect.
