加味八珍汤联合Derma-670红光治疗气血两虚型斑秃的临床疗效观察

Clinical Observation of Modified Bazhen Decoction Combined with Derma Laser-670 Irradiation Therapy in Treating Alopecia Areata with Syndrome of Deficiency of Both Qi and Blood

【目的】观察加味八珍汤联合Derma-670红光治疗气血两虚型斑秃患者的临床疗效。【方法】将62例气血两虚型斑秃患者随机分为治疗组和对照组,每组各31例。2组均给予Derma-670红光照射治疗,在此基础上,治疗组给予加味八珍汤治疗,对照组给予复方甘草酸苷片治疗,疗程为8周。观察2组患者治疗前后脱发严重程度评分(SALT)和中医证候评分的变化情况,并评价2组患者的临床疗效及安全性。【结果】(1)治疗4周后,治疗组的总有效率为67.7%(21/31),对照组为48.4%(15/31),2组疗效比较(秩和检验),差异无统计学意义(P>0.05);治疗8周后,治疗组的总有效率为93.5%(29/31),对照组为77.4%(24/31),治疗组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗4周和8周后,2组患者的SALT评分均较治疗前明显降低(P<0.01),且治疗组在治疗8周后的降低作用明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗4周和8周后,2组患者的中医证候评分均较治疗前明显降低(P<0.01),且治疗组在治疗4周和8周后的降低作用均明显优于对照组,差异均有统计学意义(P<0.01)。(4)治疗期间,2组患者的生命体征,血、尿常规和肝肾功能指标均无明显异常。【结论】加味八珍汤联合Derma-670红光治疗气血两虚型斑秃患者的临床疗效显著。

Objective To observe the clinical effect of modified Bazhen Decoction combined with Derma Laser-670 irradiation therapy in the treatment of alopecia areata with the syndrome of deficiency of both qi and blood.Methods Sixty-two alopecia areata patients with the syndrome of deficiency of both qi and blood were randomly divided into treatment group and control group,31 cases in each group. The patients in the two groups were all treated with Derma Laser-670 irradiation therapy, and additionally, the treatment group was treated with modified Bazhen Decoction and the control group was treated with Compound Glycyrrhizin Tablets. The treatment for the two groups lasted 8 continuous weeks. Before and after treatment,the scores of Severity of Alopecia Tool(SALT)and traditional Chinese medicine(TCM)syndrome in the two groups were observed. And after treatment,the clinical efficacy and safety in the two groups were also evaluated. Results(1)After treatment for 4 weeks,the total effective rate of the treatment group was 67.7%(21/31), and was higher than that of the control group(48.4%,15/31),the intergroup comparison by rank sum test showed that the difference of the clinical efficacy between the two groups being insignificant(P > 0.05). After treatment for 8 weeks,the total effective rate of the treatment group was 93.5%(29/31), and was higher than that of the control group(77.4%, 24/31), the intergroup comparison showed that the difference of the clinical efficacy between the two groups being statistically significant(P < 0.05).(2)After treatment for 4 and 8 weeks, the SALT scores of the two groups were decreased in comparison with those before treatment(P<0.01),and the treatment group was more effective on decreasing the SALT scores after treatment for 8 weeks than the control group,the difference being statistically significant(P<0.05).(3)After treatment for 4 and 8 weeks,the TCM syndrome scores of the two groups were decreased in comparison with those before treatment(P<0.01),and the treatment group was more effective on decreasing the TCM syndrome scores after treatment for 4 and 8 weeks than the control group,the difference being statistically significant(P<0.01).(4)During the treatment, no abnormalities were found in the vital signs,routine test for blood and urine,or the hepatic and renal function of the two groups. Conclusion Modified Bazhen Decoction combined with Derma Laser-670 irradiation therapy exerts certain effect for the treatment of alopecia areata with the syndrome of deficiency of both qi and blood.
