摘要
目的:探讨血小板参数与丙型肝炎患者RNA水平的相关性.方法:回顾性分析2015年1月至2019年6月检测出丙肝抗体阳性且行丙肝RNA检测的5000例HCV患者的临床资料,按照HCV-RNA载量分为HCV-RNA阴性组1603例,HCV-RNA低载量组1859例,HCV-RNA高载量组1538例,比较各组天门冬氨酸氨基转移酶水平、血小板比值指数(APRI)、血小板参数[血小板计数(PLT)、血小板分布宽度(PDW)、平均血小板体积(MPV)及血小板压积(PCT)水平].结果:HCV-RNA阳性组PDW、MPV水平及APRI评分均明显高于HCV-RNA阴性组,PLT、PCT水平均明显低于HCV-RNA阴性组,差异有统计学意义(P<0.05);HCV-RNA高载量组PDW、MPV水平及APRI评分均明显高于HCV-RNA低载量组,PLT及PCT水平均明显低于HCV-RNA低载量组,差异有统计学意义(P<0.05).结论:血小板参数与丙型肝炎患者RNA水平存在相关性,即丙型肝炎患者HCV-RNA水平越高,其PLT、PCT水平越低,而PDW、MPV水平和APRI评分越高.
Objective:To explore correlations among platelet parameters and RNA levels in patients with hepatitis C.Methods:The clinical data of 5000 HCV patients with positive hepatitis C antibodies and undergoing hepatitis C RNA test from January 2015 to June 2019 were retrospectively analyzed.According to the level of HCV-RNA load,they were divided into HCV-RNA negative group(1603 cases),low HCV-RNA load group(1859 cases)and high HCV-RNA load group(1538 cases).Then,the aspartate aminotransferase level,aspartate aminotransferase-to-platelet ratio index(APRI),and platelet parameters[platelet count(PLT),platelet distribution width(PDW),mean platelet volume(MPV)and thrombocytocrit(PCT)level]were compared among the three groups.Results:The levels of PDW and MPV as well as the APRI scores of the HCV-RNA positive group were significantly higher than those of the HCV-RNA negative group;the PLT and PCT levels were significantly lower than the HCV-RNA negative group;and the differences were statistically significant(P<0.05).The levels of PDW and MPV as well as the APRI scores of the high HCVRNA load group were significantly higher than those of the low HCV-RNA load group;the PLT and PCT levels were significantly lower than those of the low HCV-RNA load group;and the differences were statistically significant(P<0.05).Conclusions:There are correlations among the platelet parameters and RNA levels in the patients with hepatitis C.The higher the HCV-RNA level of the hepatitis C patients is,the lower their PLT and PCT levels are,and the higher their PDW,MPV levels and APRI scores are.
