罗哌卡因与布比卡因在剖宫产术腰硬联合麻醉中的应用效果比较

Comparison of effects of Ropivacaine and Bupivacaine in combined spinal-epidural anesthesia for cesarean section

目的:比较罗哌卡因与布比卡因在剖宫产术腰硬联合麻醉中的应用效果.方法:选取68例剖宫产产妇作为研究对象,采用随机数字表法将其分为对照组和观察组各34例,对照组采用布比卡因腰硬联合麻醉,观察组采用罗哌卡因腰硬联合麻醉,比较两组麻醉起效及恢复时间、麻醉效果、不良反应发生率和新生儿结局评分.结果:观察组感觉阻滞、运动阻滞起效时间均长于对照组,感觉恢复、运动恢复时间均短于对照组,差异有统计学意义(P<0.05);观察组麻醉效果Ⅰ级占比及正常新生儿占比均明显高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为2.94%(1/34),明显低于对照组的23.53%(8/34),差异有统计学意义(P<0.05).结论:罗哌卡因应用于剖宫产术腰硬联合麻醉中,可缩短麻醉恢复时间,降低不良反应发生率,提高麻醉效果,以及新生儿结局评分,其效果优于布比卡因麻醉效果,但麻醉起效时间长于布比卡因.

Objective:To compare the effects of Ropivacaine and Bupivacaine in combined spinal-epidural anesthesia for cesarean section.Methods:68 cases of cesarean section were selected as the research objects,and were divided into control group and observation group by using the random number table method,34 cases in each group.All patients underwent combined spinal-epidural anesthesia.The control group was anesthetized with Bupivacaine,while the observation group was anesthetized with Ropivacaine.The anesthesia onset time and recovery time,anesthesia effect,incidence of adverse reactions and neonatal outcome score were compared between the two groups.Results:The onset time of sensory block and motor block in the observation group was longer than that of the control group;the sensory recovery time and exercise recovery time were shorter than those of the control group;and the differences were statistically significant(P<0.05).The number of cases of grade I anesthesia and the number of normal newborns in the observation group were significantly more than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 2.94%(1/34),which was significantly lower than the 23.53%(8/34)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Ropivacaine used in combined spinal-epidural anesthesia for cesarean section can shorten the anesthesia recovery time,reduce the incidence of adverse reactions,and improve the anesthesia effect and neonatal outcome score than Bupivacaine.However,the anesthesia onset time is longer than Bupivacaine anesthesia.
