门冬胰岛素联合精蛋白锌胰岛素对妊娠期糖尿病患者妊娠结局的影响 被引量:1

Effects of insulin aspart combined with protamine zinc insulin on pregnancy outcome in patients with gestational diabetes

摘要 目的:探讨门冬胰岛素联合精蛋白锌胰岛素(NPH)对妊娠期糖尿病患者妊娠结局的影响.方法:选取80例妊娠期糖尿病患者为研究对象,采用随机数字表法分为观察组和对照组,每组40例.对照组予生物合成人胰岛素联合NPH治疗,观察组予门冬胰岛素联合NPH治疗,比较两组治疗前和分娩前血糖水平,治疗期间低血糖事件发生率及不良妊娠结局发生率.结果:分娩前,两组空腹血糖和餐后2 h血糖水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗期间低血糖事件发生率为7.50%,低于对照组的30.00%,差异有统计学意义(P<0.05);观察组不良妊娠结局发生率为15.00%,低于对照组的25.00%,差异无统计学意义(P>0.05).结论:妊娠期糖尿病患者予门冬胰岛素联合NPH治疗效果更佳,可有效调节机体血糖水平,降低低血糖事件及不良妊娠结局发生率,安全性更高. Objective:To investigate Effects of insulin aspart combined with protamine zinc insulin(NPH)on pregnancy outcome in patients with gestational diabetes.Methods:80 patients with gestational diabetes were selected as the research objects,and were divided into observation group and control group by using the random number table method,40 cases in each group.The control group was treated with biosynthetic human insulin combined with NPH,while the observation group was treated with insulin aspart combined with NPH.The blood glucose levels before the treatment and before the delivery,the incidence of hypoglycemia during the treatment and the incidence of adverse pregnancy outcomes were compared between the two groups.Results:Before the delivery,the fasting blood glucose and 2 h postprandial blood glucose levels of the two groups were lower than those before the treatment;those of the observation group were significantly lower than those of the control group;and the differences were statistically significant(P<0.05).The incidence of hypoglycemia during the treatment in the observation group was 7.50%,which was lower than 30.00%in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in the observation group was 15.00%,which was lower than 25.00%in the control group,and the difference was not statistically significant(P>0.05).Conclusions:Insulin aspart combined with NPH therapy for the gestational diabetes patients is more effective than biosynthetic human insulin combined with NPH.It can effectively regulate the body’s blood sugar level,reduce the incidence of hypoglycemia and adverse pregnancy outcomes,and has higher safety.
