摘要
血液恶性肿瘤对血小板、D-二聚体和纤维蛋白原(fibrinogen,Fbg)可能产生影响。文章采用Sys⁃mex-XN 1000全自动血液分析仪测定各组血小板参数,采用日本Sysmex CA-5100全自动血凝仪测定血浆中Fbg及D-二聚体含量,手工计数骨髓片中的巨核细胞数目,比较各检测指标在血液恶性肿瘤组与非肿瘤疾病组、正常对照组中的差异。结果发现:血液恶性肿瘤组的血小板计数(PLT)和血小板容积(PCT)小于非肿瘤疾病组和正常对照组,平均血小板体积(MPV)、大血小板比率(P-LCR)大于非肿瘤疾病组(P<0.05);血液恶性肿瘤组的Fbg及D-二聚体含量均大于非肿瘤疾病组,血液恶性肿瘤组的Fbg和D-二聚体之间呈负相关关系(P<0.05);血液恶性肿瘤组中PLT降低的患者骨髓中的巨核细胞明显少于PLT正常的患者,且PLT与D-二聚体之间呈负相关关系(P<0.05)。这些结果提示:血液恶性肿瘤对血小板、D-二聚体和Fbg均有明显影响,血液恶性肿瘤改变了患者的出凝血状态。
To investigate the effects of hematologic malignancy on platelets,D-dimer and fibrinogen(Fbg),the parameters of platelets in each group were determined by Sysmex-XN 1000 automatic hematology analyzer,and the contents of both Fbg and D-dimer in plasma were determined by Sysmex CA-5100 automatic blood coagulation analyzer.The number of megakaryocytes in bone marrow slices was counted manually.The differences of each detection index in the hematologic malignancy group,the non-tumor disease group and the normal control group were compared.Results showed that platelet count(PLT)and plateletocrit(PCT)in the hematologic malignancy group were lower than those in the non-tumor group and the normal control group,mean platelet volume(MPV)and platelet large cell ratio(P-LCR)in the hematologic malignancy group were higher than those in the non-tumor group(P<0.05).The contents of both Fbg and D-dimer in the hematologic malignancy group were higher than those in the non-tumor group,and D-dimer was negatively correlated with Fbg in the hematologic malignancy group(P<0.05).In the hematologic malignancy group,the number of megakaryocytes in bone marrow of patients with decreased PLT was significantly lower than that of patients with normal PLT,and there was a negative correlation between PLT and D-dimer(P<0.05).It demonstrates that hematologic malignancy has significant effects on platelets,D-dimer and Fbg,thus changing the state of hemostasis and coagulation in patients.
