基于CTA不同评分量表评估急性缺血性脑卒中侧支建立的研究进展

Advances in the evaluation of collateral establishment in acute ischemic stroke with different scoring scales based on CT angiography

急性缺血性脑卒中(AIS)病人缺血脑组织的侧支循环血流代偿是影响病情评估及临床预后的重要因素之一。CT血管成像(CTA)可快速、准确地评价AIS病人侧支循环血流代偿情况,从而指导治疗及判断预后。单时相、多时相及动态CTA均可以评价软脑膜侧支循环,不同的CTA方法对应不同的评估体系。对Tan评分、Alberta卒中项目早期CT评分(ASPECTS)六分量表、美国介入和治疗神经放射学学会/介入放射学学会(ASITN/SIR)侧支循环改良评分等不同侧支循环评分量表的特点、适用范围及相应基础上的研究成果转化予以综述。

Collateral circulation compensation of ischemic brain tissue in patients with acute ischemic stroke is one of the important factors affecting patients’condition evaluation and clinical prognosis.CT angiography(CTA)can be used to rapidly and correctly evaluate collateral compensation of acute ischemic stroke,thus to guide treatment and judge prognosis.Single-phase,multiphase,and dynamic CTA enable us to evaluate the collateral circulation of pia mater.Different CTA methods correspond to different evaluation systems.This article reviews the characteristics,application and research achievements of different collateral scoring scales,including Tan score,ASPECTS 6-points score,modified ASITN/SIR collateral score,etc.
