赏识理论在胃癌患者护理管理中的应用

Application of appreciation theory in nursing management of gastric cancer patients

目的探讨赏识理论在胃癌患者护理管理中的应用。方法选取2018年3月至2019年8月间安康市中心医院收治的108例胃癌患者,采用随机数字表法分为观察组和对照组,每组54例。对照组患者采用常规护理干预,观察组患者在对照组基础上采用赏识理论护理管理,比较两组患者干预前后汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、生活质量评分、干预后的治疗依从性和临床护理满意度。结果干预后,两组患者HAMA和HAMD评分均低于干预前,且观察组均低于对照组,差异均有统计学意义(均P <0.05)。干预后,观察组患者治疗依从率为90.7%,高于对照组的75.9%,差异有统计学意义(P <0.05)。干预后,两组患者认知功能、躯体功能、角色功能、情绪功能和整体功能评分均高于干预前,且观察组各评分均高于对照组,差异均有统计学意义(均P <0.05)。干预后,观察组患者对护理态度、方式、健康教育满意度及护理总满意度评分均高于对照组,差异均有统计学意义(均P <0.05)。结论胃癌患者采用赏识理论护理干预,能明显缓解患者不良情绪,提高治疗依从性、生活质量及护理满意度。

Objective To explore application of appreciation theory in nursing management of gastric cancer patients. Methods A total of 108 patients with gastric cancer who were admitted to Ankang Central Hospital from March 2018 to August 2019 were enrolled as study subjects. They were divided into an observation group and a control group by random number table method with 54 patients in each group.The observation group was given nursing management based on appreciation theory besides routine nursing and the control group was given routine nursing intervention. The scores of Hamilton anxiety scale( HAMA),Hamilton depression scale( HAMD) and quality of life before and after intervention,treatment compliance after intervention and satisfaction with clinical nursing was compared between the two groups.Results After the intervention,scores of HAMA and HAMD were significantly lower in the observation group than in the control group( all P < 0. 05). The treatment compliance rate was 90. 7% in the observation group which was higher than 75. 9% of the control group( P < 0. 05). After the intervention,scores of cognitive function,somatic function,role function,emotional function and overall function were increased in both groups with the observation group higher than the control group( all P < 0. 05). Patient satisfaction with nursing including nursing attitude,nursing patterns and health education and the overall satisfaction with nursing were higher in the observation group than in the control group( all P < 0. 05).Conclusion Nursing intervention based on appreciation theory can significantly alleviate negative emotions and improve treatment compliance,quality of life and satisfaction with clinical nursing in patients with gastric cancer.
