三孔胸腔镜与开胸肺癌根治术对老年患者近期免疫功能的影响 被引量:16

摘要 目的探讨三孔胸腔镜对比传统开胸手术对老年非小细胞肺癌患者近期免疫功能的影响。方法138例70岁及以上行肺癌根治术的非小细胞肺癌患者为研究对象,根据手术方式分为了三孔胸腔镜组和开胸组。腔镜组在胸腔镜下行三孔法肺叶切除加淋巴结清扫,开胸组患者行开胸后外侧切口肺叶切除加淋巴结清扫。分别对比两组术前、术后1 d和7 d的细胞炎性因子水平和免疫细胞含量。结果与术前相比,术后1 d两组各项细胞炎性因子的含量均明显升高(P<0.05),而组间比较无统计学意义。术后7 d,两组白细胞介素(IL)-6、C反应蛋白(CRP)均较术后1 d出现下降,其中三孔胸腔镜组下降更为明显(P<0.05)。手术后1 d两组CD4^+T和自然杀伤NK细胞水平较术前均明显下降(P<0.05)。术后7 d,NK细胞在三孔胸腔镜组恢复至术前水平(P>0.05),而开胸组术后7 d则未恢复至术前水平(P<0.05),两组组间差异有统计学意义(P<0.05);CD8^+T水平在三孔胸腔镜组术后1 d,7 d变化均不明显,差异无统计学意义(P>0.05),而开胸组在术后;CD8^+T出现了较为明显的下降,并且出现了持续性的降低。结论三孔胸腔镜肺癌根治术治疗非小细胞肺癌可明显减轻患者机体炎症反应,同时对患者机体免疫功能影响较小。
