双侧去骨瓣减压术治疗重型颅脑损伤的疗效分析

Analysis of the Curative Effect of Bilateral Decompressive Bone Decompression on Severe Craniocerebral Injury

目的对重型颅脑损伤患者采用双侧去骨瓣减压术治疗的效果进行探究。方法方便选择该院收治的重型颅脑损伤患者28例作为该次研究的纳入对象,选取时间段为:2016年6月-2018年12月。按照不同的手术治疗方法为依据将其划分为单侧组和双侧组,每组14例。采用单侧外伤大骨瓣减压术对单侧组予以治疗,采用双侧去骨瓣减压术对双侧组予以治疗,对两组患者的治疗效果进行观察,并予以比较。结果双侧组的总优良率71.4%明显高于单侧组优良率50.0%,差异有统计学意义(χ^2=8.463,P<0.05);双侧组治疗后的颅内压(16.1±4.2)mmHg明显低于单侧组(25.8±3.4)mmHg,差异有统计学意义(t=3.759,P<0.05);双侧组的并发症发生率7.1%明显低于单侧组28.6%,差异有统计学意义(χ^2=10.370,P<0.05)。结论针对重症颅脑损伤患者采用双侧去骨瓣减压术治疗具有很好的效果,可以有效地改善患者的颅内压水平,而且可以有效降低术后并发症发生率。

Objective To explore the effect of bilateral decompressive craniectomy on patients with severe craniocerebral injury.Methods 28 patients with severe craniocerebral injury in the hospital were convenient selected as the subjects of this study,and the time period was from June 2016 to December 2018.According to different surgical treatment methods,they were divided into unilateral group and bilateral group,each group had 14 cases.The patients in the two groups were treated with unilateral large trauma flap decompression and bilateral decompressive osteotomy.The therapeutic effect of the two groups was observed and compared.Results 71.4%of the total excellent rate in the bilateral group was significantly higher than 50.0%in the unilateral group,the difference was statistically significant(χ^2=8.463,P<0.05);(16.1±4.2)mmHg of the intracranial pressure in the bilateral group was significantly lower than(25.8±3.4)mmHg in the unilateral group,the difference was statistically significant(t=3.759,P<0.05);7.1%of the complications in the bilateral group was significantly lower than 28.6%in the unilateral group,the difference was statistically significant(χ^2=10.370,P<0.05).Conclusion Bilateral decompressive craniectomy for severe craniocerebral injury has a good effect,can effectively improve the level of intracranial pressure,and can effectively reduce the incidence of postoperative complications.
