乌司他丁联合奥曲肽对急性重症胰腺炎患者的临床研究

Clinical Study of Ulinastatin Combined with Octreotide in Patients with Acute Severe Pancreatitis

目的探究乌司他丁联合奥曲肽治疗急性重症胰腺炎的临床效果。方法病例选取时间:2018年11月-2019年11月,共便利选取该科室62例急性重症胰腺炎患者进行此次研究,选用电脑随机数字法将研究对象分为两组,对比组31例,研究组31例。对比组进行奥曲肽治疗,研究组进行乌司他丁联合奥曲肽治疗,分析两组的临床病症缓解时间、尿淀粉酶变化状况、并发症发生率与临床治疗效果。结果数据结果显示:研究组的临床病症缓解时间为(3.1±0.6)d,对比组为(4.6±0.8)d,差异有统计学意义(t=8.351,P=0.000<0.05);研究组的尿淀粉酶阳性率为3.26%,对比组为25.81%,差异有统计学意义(χ^2=6.369,P=0.011<0.05);研究组并发症总发生率为6.45%,对比组为29.03%,差异有统计学意义(χ^2=5.415,P=0.019<0.05);研究组的临床治疗总有效率为96.77%,对比组为80.65%,差异有统计学意义(χ^2=4.026,P=0.044<0.05)。结论在急性重症胰腺炎患者中进行乌司他丁联合奥曲肽治疗,能够更好地对患者的病症进行治疗,尽快缓解病症,降低尿淀粉酶阳性比率,且降低并发症的发生,在临床中的运用效果更好。

Objective To explore the clinical effect of ulinastatin combined with octreotide in the treatment of acute severe pancreatitis.Methods Case selection time:from November 2018 to November 2019,a total of 62 patients with acute severe pancreatitis in the undergraduate department were conveniently selected for this study.The computer random number method was used to divide the study subjects into 2 groups and 31 cases in the comparison group.There were 31 cases in the study group.The comparison group was treated with octreotide.The study group was treated with ulinastatin combined with octreotide.Results The results of the data show that:the time for remission of the clinical symptoms in the study group was(3.1±0.6)d,and that in the comparison group was(4.6±0.8)d.The difference was statistically significant(t=8.351,P=0.000<0.05).The positive rate of urine amyloase in the study group was 3.26%,the comparison group was 25.81%,the difference was statistically significant(χ^2=6.369,P=0.011<0.05).The total incidence of complications in the study group was6.45%,and the comparison group was 29.03%,the difference was Statistical significance(χ^2=5.415,P=0.019<0.05).The total effective rate of clinical treatment in the study group was 96.77%,and the comparison group was 80.65%,the difference was statistically significant(χ^2=4.026,P=0.044<0.05).Conclusion Ulinastatin combined with octreotide treatment in patients with acute severe pancreatitis can better treat the patients’symptoms,relieve the symptoms as soon as possible,reduce the rate of urinary amylase positiveness,and reduce the incidence of complications.The application effect is better.
