急性阑尾炎采用腹腔镜阑尾切除术和传统开腹阑尾切除术治疗的对比研究

A Comparative Study on the Treatment of Acute Appendicitis with Laparoscopic Appendectomy and Traditional Open Appendectomy

目的研究对比传统开腹阑尾切除术与腹腔镜阑尾切除术治疗急性阑尾炎的临床效果。方法通过随机化原则将随机选取该院2018年1月—2019年8月收治的60例急性阑尾炎患者分为两组,对照组(n=30)行传统开腹阑尾切除术,治疗组(n=30)行腹腔镜阑尾切除术。对比两组手术指标、疼痛评分与术后并发症发生率。结果手术时间:治疗组(66.18±21.86)min,对照组(54.75±19.87)min(t=2.119,P=0.038);术中出血量:治疗组(12.81±1.55)mL,对照组(27.99±2.30)mL(t=29.977,P=0.000);术后排气时间:治疗组(20.12±9.76)h,对照组(27.64±10.36)h(t=2.894,P=0.005);住院时间:治疗组(5.49±2.02)d,对照组(8.71±2.23)d(t=5.862,P=0.000)。治疗后疼痛评分:治疗组(1.00±0.04)分,对照组(2.09±0.29)分(t=20.394,P=0.000)。术后并发症方面:治疗组6.67%,对照组26.67%(χ2=4.320,P<0.05)。结论腹腔镜阑尾切除术治疗急性阑尾炎安全高效,既能优化手术指标、减轻疼痛,又可减少术后并发症、提高手术疗效,临床推广价值高。

Objective To compare the clinical effects of traditional laparoscopic appendectomy and laparoscopic appendectomy on acute appendicitis.Methods The 60 patients with acute appendicitis admitted to the hospital from January 2018 to August 2019 were randomly eslected and divided into two groups by randomization.Conventional open appendectomy was performed in the control group(n=30)and laparoscopic appendectomy was performed in the treatment group(n=30).The surgical indexes,pain scores and postoperative complications were compared between the two groups.Results Operation time,treatment group(66.18±21.86)min,control group(54.75±19.87)min(t=2.119,P=0.038);intraoperative blood loss,treatment group(12.81±1.55)mL,control group(27.99±2.30)mL(t=29.977,P=0.000);postoperative exhaust time,treatment group(20.12±9.76)h,control group(27.64±10.36)h(t=2.894,P=0.005);hospital stay,treatment group(5.49±2.02)d,control group(8.71±2.23)d(t=5.862,P=0.000).After treatment,the pain score was divided into the treatment group(1.00±0.04)points and the control group(2.09±0.29)points(t=20.394,P=0.000).In terms of postoperative complications,the treatment group was 6.67%and the control group was 26.67%(χ2=4.320,P<0.05).Conclusion Laparoscopic appendectomy is safe and effective for the treatment of acute appendicitis,which can not only optimize the surgical index,reduce pain,but also reduce postoperative complications,improve the surgical efficacy,and have high clinical popularization value.
