摘要
目的本研究通过分析广州地区学龄儿童原发性高尿酸血症常见的危险因素,并通过超声比较其与同龄健康儿童的肾动脉血流特征、颈总动脉内-中膜厚度,研究学龄儿童高尿酸血症早期血管的变化,为学龄儿童原发性高尿酸血症的早期干预及治疗提供帮助。方法选择2018年6月至2019年6月在广州市番禺区中心医院体检的年龄为6~12岁的学龄期儿童共112例为研究对象,其中高尿酸组40例,健康组72例。对研究对象进行身高、体重、血压检测和问卷调查,并比较两组对象的血常规、尿常规、肝肾功能、血清尿酸、双肾及肾动脉彩色多普勒超声、颈动脉彩色多普勒超声检查等结果。结果高尿酸血症的学龄儿童较健康儿童更喜欢吃零食及夜宵(P<0.05)。肥胖或超重者、直系家族有高尿酸血症者的学龄儿童发生高尿酸血症的概率大于健康者(P<0.05)。高尿酸组的左肾叶间动脉阻力指数(RI)较健康组增高(P<0.01)。高尿酸组两侧颈总动脉内-中膜厚度较健康组增厚(P<0.05)。结论学龄儿童肥胖及不健康的饮食习惯是导致原发性高尿酸血症的原因之一。高尿酸血症所致的肾脏小动脉的早期改变可能导致肾小动脉的血流阻力升高。早年的高尿酸血症如不及时处理,可能导致成年后血管的内-中膜增厚到粥样硬化的变化。
Objective To analyze the common risk factors for primary hyperuricemia in school age children in Guangzhou,and to compare the characteristics of renal blood flow and the innermedia thickness of carotid artery.To study the changes of early arterial disease who have hyperuricemi aons in school-age.So the study can help us to intervene and treat earlier of primary hyperuricemia in school-age.Methods The subjects of 112 children age from 6 to 12 years whose routine medical examination were in department of Panyu central hospital from June 2018 to June 2019.Among them,40 were in the hyperuricemia group and 72 in the healthy group.Medical examination included:height,weight,blood pressure,questionnaire,blood routine,urine routine,liver and kidney function,serum uric acid,color Doppler ultrasound of renal and renal artery,color Doppler ultrasond of carotid artery.Results School-age children with hyperuricemia prefered snacks and loved eating at night compared with healthy group(P<0.05).School-age children with obesity or overweight,or immediate family with hyperuricemia had a higher incidence of hyperuricemia than healthy group(P<0.05).The RI of the left interlobular artery in the group of hyperuricemia were higher than that group of healthy(P<0.01).The intima media thickness of the carotid artery were thicker in the group of hyperuricemia(P<0.05).Conclusions Obesity and unhealthy eating habits in schoolge children are the causes of hyperuricemia.Early changes in renal arterioles by hyperuricemia will increase the obstruction of blood flow in the renal arterioles.If not treats in time of early hyperuricemia,it can cause the intima-media thickness of blood vessels to thicken or even harden.
