血清内皮素-1联合血红蛋白对心肌梗死患者经皮冠状动脉介入术近期预后的预测效能研究

Predictive Efficacy of Serum Endothelin-1 Combined With Hemoglobin in The Short-Term Prognosis of Patients with Myocardial Infarction after Percutaneous Coronary Intervention

目的探讨血清内皮素-1(ET-1)联合血红蛋白(Hb)对心肌梗死患者经皮冠状动脉介入术(PCI)近期预后的预测效能。方法选择心肌梗死患者220例,检测所有患者的血清ET-1、Hb值,调查患者的预后并进行预测效能分析。结果所有患者都顺利完成PCI治疗,随访至2020年2月,预后不良45例,占比20.5%,其中全因死亡6例、靶血管再重建24例、非致死性心肌梗死10例、心衰5例。两组病变支数与病变分级对比差异有统计学意义(P<0.05)。预后不良组的血清ET-1值与Hb含量都显著高于预后良好组(P<0.05)。直线相关分析显示病变支数、病变分级、ET-1、Hb都与预后不良呈相关性(P<0.05);非条件Logistic回归分析显示病变支数、病变分级、ET-1、Hb都为影响患者预后的因素(P<0.05)。结论心肌梗死PCI治疗后近期预后不良患者多伴随有血清ET-1和Hb的高表达,血清ET-1联合Hb可成为心肌梗死患者预后的预测因子之一。

Objective To explore and study the predictive efficacy of serum ET-1 combined with Hb on the short-term prognosis of patients with myocardial infarction after PCI.Methods 220 cases of patients with myocardial infarction were selected.Serum ET-1 and Hb values of all patients were detected,the prognosis of the patients and the predictive efficacy were analyzed.Results All patients were successfully completed the PCI treatment.Up to now,there were 45 patients were in poor prognosis,accounted for 20.5%,of which 6 patients were all-cause deaths,24 patients were revascularization of target vessels,10 patients were nonfatal myocardial infarction,and 5 patients were heart failure.There were statistically significant difference in the number of lesions and the grade of the disease compared between the two groups(P<0.05).The serum ET-1 value and Hb content in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.05).Linear correlation analysis showed that the number of lesions,lesion grade,ET-1,and Hb were correlated with poor prognosis(P<0.05);Unconditional logistic regression analysis showed that the number of lesions,lesion grade,ET-1,and Hb were all affected.Prognostic factors of patients(P<0.05).Conclusion Patients with poor prognosis in the short term after myocardial infarction are often accompanied by high expression of serum ET-1 and Hb.Serum ET-1 combined with Hb can be one of the predictors of the prognosis of patients with myocardial infarction.
