超声乳化及青光眼白内障联合手术治疗合并白内障的闭角型青光眼疗效比较研究

Comparison of effectiveness of phacoemulsification and combined glaucoma-cataract surgery in closed angle glaucoma with cataract

目的分析超声乳化及青光眼白内障联合手术治疗合并白内障的闭角型青光眼疗效比较研究。方法选取2018年1月至2018年10月阳江市阳江光明眼科医院收治的80例(80眼)合并白内障的闭角型青光眼患者,根据住院次序随机分为观察组和对照组,每组40例(40眼)。观察组采用超声乳化术治疗;对照组采用青光眼白内障联合手术治疗。观察比较两组临床治疗效果、视力水平及眼压改善情况。结果观察组治疗后总有效率高于对照组(97.50%vs. 77.50%)(P<0.05)。观察组术后视力水平、前房深度较对照组高(P<0.05);而眼压水平较对照组低(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论超声乳化术及青光眼白内障手术治疗合并白内障的闭角型青光眼均可治疗合并白内障闭角型青光眼。超声乳化术可以达到较高的成功率,且术后通常不会出现较多的并发症,具有更为明显的治疗效果,对患眼恢复促进作用明显,具有更高的临床应用价值。

【Objective】 To compare the efficacy of phacoemulsification and glaucoma combined with cataract surgery for angle-closure glaucoma.【Methods】Eighty patients(80 eyes) with closed-angle glaucoma complicated with cataract admitted to our hospital from January 2018 to October 2018 were randomly assigned to the observation group and the control group according to the order of admission, with 40 patients(40 eyes) in each group. The observation group was treated with phacoemulsification, and the control group was treated with combined glaucoma-cataract surgery. The clinical treatment effect and improvement of visual acuity and intraocular pressure were observed. 【Results】 After treatment, the total effective rate of the observation group was 97.50%,which was significantly higher than that of the control group(77.50%)(P<0.05). The visual acuity in the observation group(0.70±0.07) was significantly higher than that in the control group(0.34±0.05)(P<0.05). The level of intraocular pressure in the observation group(11.99±1.71 mmHg) was significantly lower than that in the control group(15.32±2.48 mmHg)(P<0.05). The anterior chamber depth in the observation group(3.58±0.21 mm) was significantly lower than that in the control group(3.10±0.36 mm)(P<0.05). 【Conclusion】 Ultrasonic emulsification cataract and combined glaucoma-cataract surgery can be a one-time treatment for angle-closure glaucoma with cataract. Ultrasonic emulsification for cataract and intraocular lens implantation can achieve a higher success rate, and usually there will be no more postoperative complications;it has more obvious treatment effect,obvious promoting effect for eye restore, and higher clinical value.
