摘要
目的探讨妊娠晚期孕妇B族溶血性链球菌(GBS)感染诊断中应用实时聚合酶链反应(real-time PCR)的价值。方法选取2018年8月至2019年8月在濮阳市妇幼保健院产检的380例妊娠晚期孕妇。所有孕妇入院时采集阴道及直肠分泌物,进行real-time PCR法与细菌培养法检测。GBS感染诊断金标准为两种方法联合诊断结果,比较两种诊断方法阳性检出情况,并分析其诊断的敏感性、特异性和准确性。结果 380例妊娠晚期孕妇中,经real-time PCR法联合细菌培养法诊断有30例GBS感染者,阳性率为7.89%;real-time PCR法诊断有29例GBS感染者,阳性率为7.63%;细菌培养法诊断有9例GBS感染者,阳性率为2.37%。real-time PCR法阳性检出率高于细菌培养法,差异有统计学意义(P<0.05)。real-time PCR法检测的敏感性、特异性和准确性较细菌培养法高,差异有统计学意义(P<0.05)。结论妊娠晚期孕妇GBS感染诊断中应用real-time PCR法的诊断效能高,可为临床诊疗提供可靠依据。
【Objective】 To investigate the value of real-time PCR in the diagnosis of group B hemolytic streptococcus(GBS)infection in late pregnancy.【Methods】 A total of 380 pregnant women in the third trimester of pregnancy recieved prenatal checkup from August 2018 to August 2019 in our hospital were selected. Vaginal and rectal secretions were collected from all pregnant women when they were admitted to the hospital for real-time PCR and bacterial culture detection. The "gold standard" of GBS infection diagnosis was the combined diagnosis results of the two methods. The positive detection of the two methods was compared, and the sensitivity, specificity and accuracy of the diagnosis were analyzed.【Results】 Among 380 pregnant women in the third trimester of pregnancy, there were 30 cases with GBS infection diagnosed by real-time PCR combined with bacterial culture, the positive rate was 7.89%;29 cases with GBS infection diagnosed by real-time PCR, the positive rate was 7.63%;9 cases with GBS infection diagnosed by bacterial culture, the positive rate was 2.37%. The positive detection rate of real-time PCR was higher than that of bacterial culture, and the difference was statistically significant(P<0.05). Compared with culture method, the sensitivity, specificity and accuracy of real-time PCR were higher, and the difference was statistically significant(P<0.05).【Conclusion】The application of real-time PCR in the diagnosis of GBS infection in late pregnancy has a high diagnostic efficiency and can provide a reliable basis for clinical diagnosis and treatment.
