右美托咪啶对肺癌根治术患者围手术期外周血淋巴细胞亚群的影响

Effect of dexmedetomidine on perioperative peripheral blood lymphocyte in patients with lung cancer after radical resection

目的:观察右美托咪啶对肺癌根治术患者围手术期外周血淋巴细胞亚群的影响以反映对机体免疫功能的影响。方法:选取2017年1月至2019年1月我院98例行胸腔镜肺癌根治术的患者进行研究,根据围手术期使用右美托咪啶情况分为研究组和对照组。麻醉诱导前,研究组静脉输注右美托咪啶,对照组使用同等剂量生理盐水。分别于手术前(T0)、手术结束时(T1)及术后第三天(T2)检测患者T淋巴细胞、B淋巴细胞、NK细胞水平。利用统计学软件SPSS 25.0进行数据处理,分析右美托咪啶对患者免疫功能的影响。结果:两组患者性别、年龄、体质指数及肿瘤部位等一般资料无明显差异(P>0.05);T1时研究组和对照组免疫功能均明显低于T0时,差异具有统计学意义(P<0.05);T2时研究组与对照组相比,研究组的免疫功能明显强于对照组,差异具有统计学意义(P<0.05)。结论:围手术期使用右美托咪定对肺癌根治术患者免疫功能的改善作用,可以为右美托咪定在肿瘤患者全身麻醉中的安全应用提供理论依据,进而改善肿瘤患者的疗效及预后。

Objective:To observe the effect of dexmedetomidine on perioperative peripheral blood lymphocytes in patients with radical lung cancer,reflecting the impact on immune function.Methods:A total of 98 patients with lung cancer after radical resection in our hospital from January 2017 to January 2019 were enrolled.The study was divided into research group and control group according to the perioperative period using dexmedetomidine.Before the induction,the research group received dexmedetomidine intravenously,and the control group did not use dexmedetomidine.The levels of T lymphocytes,B lymphocytes and NK cells were measured before surgery(T0),at the end of surgery(T1)and on the third day after surgery(T2).Data processing was performed using statistical software SPSS 25.0 to analyze the effect of dexmedetomidine on immune function of patients.Results:There were no significant differences in gender,age,body mass index and tumor location between the two groups(P>0.05).At T1,the immune function of the research group and the control group were significantly lower than T0,and the difference was significant(P<0.05).Compared with the control group at T2,the immune function of the research group was significantly stronger than that of the control group,and the difference was significant(P<0.05).Conclusion:Perioperative use of dexmedetomidine for the improvement of immune function in patients with lung cancer radical surgery,can provide a theoretical basis for the safe application of dexmedetomidine in general anesthesia of tumor patients,thereby improving the efficacy and prognosis of patients with cancer.
