新型冠状病毒感染14例早期临床特征分析

Clinical characteristics of SARS-CoV-2 infection at early stage

目的分析重庆市大足区人民医院新型冠状病毒感染者的早期临床特征。方法回顾性收集2020年1-2月住院且入院后经核酸检测明确诊断为新型冠状病毒感染患者的临床资料,均于入院当天行流行病学资料采集及血常规、肝肾功能、胸部CT、血气分析、呼吸道病毒抗原、甲型/乙型流感病毒抗原、凝血功能、降钙素原等检测;并依据《新型冠状病毒肺炎诊疗方案(试行第五版)》进行分析。结果大足地区共确诊14例患者,平均年龄(53.5±12.9)岁,男性9例,女性5例;1例患者有武汉居住史,12例为密切接触感染者中有10例家庭聚集性病例,1例患者无新型冠状病毒感染者或周围发热、咳嗽患者接触史;平均潜伏期(7.0±4.0)d,最短2 d,最长14 d;患者多表现为干咳(10例)、乏力(6例)、发热(4例),胸闷、恶心、心慌、咽痒、畏寒等症状在不同患者中有所表现,1例患者以腹泻就诊;胸部CT多表现双肺多发磨玻璃影(12例),2例患者胸部CT正常;9例普通型病例,2例轻型病例,2例重型病例,1例危重型病例。结论大足地区新型冠状病毒感染以家庭聚集性病例为主;患者以干咳、乏力、发热为主要表现;胸部CT多表现为双肺多发磨玻璃样影;感染新型冠状病毒后多数为普通病例。

Objective To describe the clinical features of patients infected with SARS-CoV-2 in Dazu District People’s Hospital of Chongqing.Methods Fourteen patients infected with SARS-CoV-2 were treated in Dazu District People’s Hospital of Chongqing from January 2020 to February 2020.The epidemiological data,blood tests,renal and liver function tests,chest CT images,blood gas analysis,respiratory virus antigen,coagulation function,influenza A/B virus antigen,procalcitonin were collected at admission,analyzed according to the Diagnosis and Treatment Protocol for SARS-CoV-2-infected Pneumonia(Fifth Edition).Results Fourteen cases were confirmed in this study,including 9 males and 5 females.The average age of the patients was(53.5±12.9)years.One patient had a history of living in Wuhan.Of these 14 patients,12 were infected by close contact with confirmed patients.Ten cases belonged to family cluster.Notably,one patient had neither the history of living or visiting Wuhan and the surrounding places nor the history of contacting with any confirmed patients.The mean incubation period was(7.0±4.0)days(range from 2 to 14 days).Dry cough(10 cases),fatigue(6 cases),and fever(4 cases)were the most common symptoms.Other symptoms included nausea,chest distress,shortness of breath,chills,and pharyngeal itching.Notably,one patient came to our fever clinic with diarrhea.Multiple ground glass opacities were found in bilateral lungs on chest CT images in 12 cases.Chest CT images were normal in the remaining two cases.Nine cases were clinically classified as moderate type;two each as severe and mild type,respectively.Only one case progressed to critical type.Conclusions The patients infected with SARS-CoV-2 in this report were mainly from family clusters.Fever,fatigue,and dry cough were the most common symptoms.Majority of the patients were moderate type after infection.
