探讨经阴道彩色多普勒超声在子宫附件囊性占位性病变中的临床作用

To investigate the clinical effect of transvaginal color doppler ultrasound(TCD)in cystic lesion of uterine appendage

目的:分析经阴道彩色多普勒超声在子宫附件囊性占位性病变中的应用价值。方法:本研究86例子宫附件囊性占位性病变患者均为本院2018年1月至2018年12月接收,所选患者均进行经阴道彩色多普勒超声诊断,分析诊断结果。结果:经手术病理检验,确诊病例54例,其中8例误诊,超声诊断与病理检测符合率为85.19%;本研究卵巢非赘生性囊肿患者29例,囊性占位平均直径(4.52±1.65)cm。患者月经5~7d后实施超声复查,进行连续6个月检测,其中出血性黄体囊肿跟踪2个月后消失9例;卵泡囊肿跟踪3个月消失15例;跟踪随访6个月,囊性占位大小未见显著改变5例,其中经手术确诊为卵巢单纯性囊肿3例。超声诊断符合率89.66%。本研究中,3例经阴道超声诊断确诊为输卵管积液,经随访可知,显著缩小2例,经手术复查,1例明确诊断为卵巢单纯性囊肿。结论:子宫附件囊性占位性病变诊断过程中,经阴道彩色多普勒超声应用价值高,除清楚显示病灶外,可为临床诊治提供数据参考。

Objective:To analyze the application value of transvaginal color doppler ultrasound in cystic lesion of uterine adnexa.Methods:In this study,86 patients with cystic space-occupying lesions in the uterine adnexa were admitted to our hospital from January 2018 to December 2018.All the patients were diagnosed by transvaginal color doppler ultrasound and the diagnostic results were analyzed.Results:After pathological examination,54 cases were confirmed,8 of which were misdiagnosed.The coincidence rate between ultrasonic diagnosis and pathological examination was 85.19%.In this study,29 patients with ovarian non-vegetative cysts were studied,with a mean cystic site diameter of(4.52±1.65)cm.Ultrasound reexamination was carried out after 5~7 days of menstruation for 6 consecutive months,among which 9 cases of hemorrhagic luteal cyst disappeared after 2 months of follow-up.Follicular cyst disappeared after 3 months in 15 cases.Follow-up of 6 months showed no significant change in the size of cystic mass in 5 cases,among which 3 cases were diagnosed as simple ovarian cyst by surgery.The coincidence rate of ultrasonic diagnosis was 89.66%.In this study,3 cases were diagnosed as oviduct effusion by transvaginal ultrasound,2 cases were significantly reduced by follow-up,and 1 case was definitely diagnosed as simple ovarian cyst after surgical review.Conclusion:Transvaginal color doppler ultrasound is of high value in the diagnosis of cystic space occupying lesions in uterine appendages.
