摘要
目的系统评价人端粒酶逆转录酶(hTERT)在鉴别良恶性乳腺肿瘤中的临床价值。方法检索中国生物医学文献服务系统、中国知网、万方、维普、PubMed、The Cochrane Library、Web of Science、EMbase数据库中有关hTERT检测与乳腺肿瘤良恶性相关的文章,检索时间均从建库至2019年10月9日。采用Stata15.0和Meta-Disc1.4软件进行统计分析。结果共纳入17个研究,共计1436例患者(恶性肿瘤组852例,对照组584例)。结果显示,hTERT诊断乳腺恶性肿瘤的合并灵敏度(Sen合并)、合并特异度(Spe合并)、合并阳性似然比(+LR合并)、合并阴性似然比(-LR合并)和合并诊断比值比(DOR合并)分别为0.81(95%CI:0.79~0.84)、0.83(95%CI:0.79~0.86)、4.75(95%CI:3.35~6.74)、0.21(95%CI:0.15~0.30)、26.93(95%CI:15.02~48.28);汇总受试者工作特征(SROC)曲线下面积(AUC)=0.9065,Q*=0.8383。结论hTERT检测有助于临床鉴别乳腺肿瘤良恶性。
Objective To systematically evaluate the clinical value of human telomerase reverse transcriptase(hTERT)in the differentiation of benign and malignant breast tumors.Methods A computer based online search was conducted in PubMed,the Cochrane Library,Web of Science,EMbase,CBM,CNKI,WanFang,VIP database from the establishment of the databases to October 9,2019 to collect the relevant diagnostic studies of hTERT for benign and malignant breast tumors.Then screened literatures,extracted datas and assessed the methodological quality of included studies.Meta-analysis was performed by Stata 15.0 and Meta-Disc 1.4 softwares.Results A total of 17 studies involving 1436 patients(852 cases in the malignant tumor group and 584 cases in the control group)were finally included.Meta-analysis showed that the combined sencombined,combined specombined,-LRcombined,+LRcombined,and DORcombined of hTERT in differentiation of bengin and mailgnant breast tumors were 0.81(95%CI:0.79-0.84),0.83(95%CI:0.79-0.86),4.75(95%CI:3.35-6.74),0.21(95%CI:0.15-0.30),26.93(95%CI:15.02-48.28),respectively.The AUC of ROC was 0.9065,Q*=0.8383.Conclusion hTERT detection is helpful to distinguish benign and malignant breast tumors.
