胆囊神经内分泌癌的CT表现分析

Analysis of CT manifestations of neuroendocrine carcinoma of gallbladder

目的探讨分析胆囊神经内分泌癌的CT影像学表现特点,提高对罕见病的认识。方法回顾性分析我院经病理证实的3例胆囊神经内分泌癌的CT检查结果、病理和临床资料,其中1例同时行MRI检查。观察肿块的分布范围、大小及强化方式,是否合并局部浸润、淋巴结转移,有无胆囊结石。结果3例胆囊神经内分泌癌CT平扫时表现为胆囊壁的不规则弥漫性增厚或胆囊窝的团块,增强扫描为不均匀强化。2例合并有肝脏和淋巴结转移,1例合并有肝总管浸润。结论胆囊神经内分泌癌CT表现为胆囊部位的弥漫性肿块,易侵犯邻近的肝脏组织,伴有胆管侵犯、肝门部、腹膜后淋巴结转移。

Objective To explore and analyze the CT imaging features of neuroendocrine carcinoma of the gallbladder to improve the understanding of this rare disease.Methods The CT findings,pathology and clinical data of 3 cases of neuroendocrine carcinoma of the gallbladder confirmed by pathology in our hospital from January 2010 to June 2020 were retrospectively analyzed,among which 1 case underwent MRI examination simultaneously.We observed the distribution range,size and enhancement method of the tumor,whether it was combined with local infiltration,lymph node metastasis,and whether there were gallbladder stones.Results 3 cases of neuroendocrine carcinoma of the gallbladder showed irregular diffuse thickening of the gallbladder wall or masses of the gallbladder fossa during CT scan,and the enhanced scan showed uneven enhancement.2 cases had liver and lymph node metastasis,and 1 case had common hepatic duct infiltration.Conclusion CT findings of neuroendocrine carcinoma of the gallbladder are diffuse masses in the gallbladder,easily invading the adjacent liver tissue,accompanied by bile duct invasion,hilar and retroperitoneal lymph node metastasis.
