后牙邻面龋不同树脂填充法的效果及对患者术后牙周炎发生的影响

The Effect of Different Resin Filling Methods on Posterior Approximal Caries and Its Effect on Patients with Periodontitis

目的:探讨后牙邻面龋常规树脂修补法及树脂球修补法的治疗效果及对术后牙周炎发生的影响。方法:选择2018年10月-2019年3月于笔者医院治疗的后牙邻面龋患者120例。按照随机数字表法将所有患者分为树脂球组(n=60)和常规组(n=60),两组患者常规行龋洞清除术后使用光固化树脂法对龋洞进行填充,树脂球组在此基础上加行树脂球楔形填充法,术后随访6个月。比较两组患者治疗后及末次随访时疗效及术处相邻牙接触情况,治疗前、治疗后及末次随访时牙周健康相关指标,治疗后不良反应、随访期间食物嵌塞及填充物脱落的情况。结果:树脂球组治疗后接触关系不满意占比为31.57%,显著低于常规组的52.73%(P<0.05);树脂球组患者末次随访时探诊深度及探诊出血指数显著低于常规组(P<0.05);两组患者治疗后不良反应发生率分别为7.02%和5.54%,组间比较无显著性差异(P>0.05)。树脂球组患者食物嵌塞发生率为1.8%,显著低于对照组的12.7%(χ^2=5.077,P=0.024)。两组患者填充物脱落发生率分别为3.5%和10.9%,组间比较无显著性差异(χ^2=2.359,P=0.125)。结论:树脂球填充法能有效改善后牙邻面龋患者龋洞及填充后邻面接触情况,从而避免治疗部位牙周炎性反应的发生,进一步优化治疗效果。

Objective To investigate the effect of conventional resin repair and resin ball repair on the posterior approximal caries and the difference in the effect on periodontitis after surgery.Methods A total of 120 patients with posterior dental caries treated in our hospital from October 2018 to March 2019 were selected.All patients were divided into a resin ball group(n=60)and a conventional group(n=60)according to the random number table method.The two groups of patients were routinely filled with caries cavity using light curing resin method after caries cavity removal.The resin ball group was added with resin ball wedge filling method and followed up for 6 months.The efficacy of the two groups were compared after treatment and at the last follow-up,and the contact status of adjacent teeth at the operation site was compared.The indicators of periodontal health at the operation site were compared before,after and at the last follow-up.The adverse reactions after treatment,food impaction and filling shedding during the follow-up were compared between the two groups of patients.Results The proportion of unsatisfactory contact relationships in the resin ball group after treatment was 31.57%,which was significantly lower than 52.73%in the conventional group(P<0.05).Patients in the resin ball group had significantly lower depth of exploration and exploratory bleeding index at the last follow-up than those in the conventional group(P<0.05).The incidence of adverse reactions after treatment in the two groups was 7.02%and 5.54%,respectively,and there was no significant difference between the groups(P>0.05).The incidence of food impaction in the resin ball group was 1.8%,which was significantly lower than the control group's 12.7%(χ^2=5.077,P=0.024).The incidence of filler shedding in the two groups was 3.5%and 10.9%,respectively,and there was no significant difference between the groups(χ^2=2.359,P=0.125).Conclusion The resin ball filling method can effectively improve the caries cavity of patients withposterior approximal caries and the contact between adjacent teeth after filling,thereby avoiding the occurrence of periodontal inflammatory reactions at the operation site and further optimizing the treatment effect.
