认知干预配合心理干预乳腺癌术后患者PICC置管相关并发症及情绪的影响

Effects of cognitive intervention and psychological intervention on puncture the central vein through the peripheral vein catheterization related complications and mood in patients with breast cancer

目的探究认知干预配合心理干预乳腺癌术后患者经外周静脉穿刺中心静脉(PICC)置管相关并发症及情绪的影响。方法选取2018年10月—2019年12月陕西省榆林市第二医院收治的52例乳腺癌术后超声引导下PICC置管患者,将其分为对照组(常规护理干预)和观察组(常规护理干预基础上强化认知和心理干预),每组26例,比较两组焦虑、抑郁情况,以及PICC置管相关并发症发生率和护理满意度。结果观察组干预后焦虑自评量表和抑郁自评量表评分均较干预前下降,且观察组均明显低于对照组(P<0.05)。观察组PICC置管相关并发症发生率低于对照组(P<0.05)。观察组护理干预满意度显著高于对照组(P<0.05)。结论乳腺癌术后患者的护理干预中,行认知干预配合心理干预,对患者焦虑、抑郁情绪的改善有十分重要的作用,可降低PICC置管相关并发症发生率,提升患者的护理满意度。

Objective To explore the effects of cognitive intervention and psychological intervention on the complications and emotions associated with puncture the central vein through the peripheral vein(PICC)in patients with breast cancer after peripheral venipentesis.Methods From October 2018 to December 2019,52 patients with ultrason-guided PICC catheter after breast cancer surgery in the Second Hospital of Yulin City were selected and divided into control group(routine nursing intervention)and observation group(enhanced cognitive and psychological intervention based on routine nursing intervention),with 26 patients in each group.Anxiety,depression,incidence of PICC catheter-related complications and nursing satisfaction were compared between the two groups.Results After intervention,the scores of anxiety self-rating scale and depression self-rating scale in the observation group decreased compared with that before intervention,and the scores of the observation group were significantly lower than those of the control group(P<0.05).The incidence of PICC catheter-related complications in the observation group was lower than that in the control group(P<0.05).Nursing intervention satisfaction in the observation group was significantly higher than that in the control group(P<0.05).Conclusion In the nursing intervention for breast cancer patients after operation,cognitive intervention combined with psychological intervention plays a very important role in the improvement of anxiety and depression in patients,which can reduce the incidence of PICC catheter-related complications catheterization and improve the nursing satisfaction of patients.
