微创经皮椎弓根钉手术与传统后路开放手术治疗胸腰椎骨折的临床疗效对比

Comparison of clinical efficacy on minimally invasive percutaneous pedicle screw surgery and conventional posterior open surgery in the treatment of thoracolumbar fractures

目的:对比微创经皮椎弓根钉手术与传统后路开放手术治疗胸腰椎骨折的临床疗效。方法:2018年10月-2019年10月收治胸腰椎骨折患者46例,随机分为两组,各23例。研究组实施微创经皮椎弓根钉内固定术治疗;对照组实施传统后路开放椎弓根钉内固定术治疗。比较两组治疗效果。结果:研究组手术时间、术中出血量、术后下床时间及出院时间均低于对照组,差异有统计学意义(P<0.05)。两组术后椎体前缘、后缘高度及Cobb角均明显均优于术前,差异有统计学意义(P<0.05)。两组均未出现相关并发症。结论:微创经皮椎弓根钉手术与传统后路开放手术治疗胸腰椎骨折均有满意治疗效果和较高的安全性,但微创经皮椎弓根钉手术时间短,出血少,术后恢复快,患者满意度更高。

Objective:To compare the clinical efficacy of minimally invasive percutaneous pedicle screw surgery and conventional posterior open surgery in the treatment of thoracolumbar fractures.Methods:From October 2018 to October 2019,46 patients with thoracolumbar fracture were selected and they were randomly divided into the two groups with 23 patients in each group.Minimally invasive percutaneous pedicle screw fixation was performed in the study group.The control group was treated with conventional posterior open pedicle screw fixation.The therapeutic effects between the two groups was compared.Results:The surgical time,intraoperative blood loss volume,postoperative time of getting out of bed and discharge time in the study group were all lower than the control group,with the differences were statistically significant(P<0.05).The leading edge,rear edge height and Cobb angle of postoperative were significantly better than before surgery in both groups,with the differences were statistically significant(P<0.05).There were no related complications in either group.Conclusion:Minimally invasive percutaneous pedicle screw surgery and conventional posterior open surgery for thoracolumbar fractures have satisfactory treatment effect and high safety,however,minimally invasive percutaneous pedicle screw surgery has shorter time,less bleeding,faster postoperative recovery,and higher patient satisfaction.
