托伐普坦联合小剂量多巴胺治疗顽固性心力衰竭的临床观察

Clinical observation on the tolvaptan combined with small doses of dopamine in the treatment of stubborn heart failure

目的:分析托伐普坦联合小剂量多巴胺治疗顽固性心力衰竭的临床疗效。方法;2017年1月-2019年8月收治顽固性心力衰竭患者100例,随机分为两组,各50例。对照组给予托伐普坦治疗;观察组给予托伐普坦联合小剂量多巴胺治疗。比较两组各项临床指标水平。结果:观察组治疗后血钠、血钾、尿肌酐及尿素等指标水平均显著优于对照组,差异有统计学意义(P<0.05)。观察组心肌肌钙蛋白、N末端脑钠肽及超敏C-反应蛋白等指标水平均低于对照组,差异有统计学意义(P<0.05)。观察组呼吸困难及全身状况评分均显著优于对照组,差异有统计学意义(P<0.05)。结论:托伐普坦联合小剂量多巴胺治疗顽固性心力衰竭,效果显著。

Objective:To analyze the clinical efficacy of tolvaptan combined with small dose of dopamine in the treatment of stubborn heart failure.Methods;From January 2017 to August 2019,100 patients with stubborn heart failure were selected and they were randomly divided into the two groups with 50 patients in each group.The control group was treated with tolvaptan.The observation group was treated with tolvaptancombined with small dose of dopamine.The levels of various clinical indicators were compared between the two groups.Results:The indicators levels of serum sodium,serumpotassium,urinary creatinine and urea were significantly better in the observation group than the control group,with the differences were statistically significant(P<0.05).The indicators levels of cardiac troponin,N-terminal brain natriuretic peptide and hypersensitive C-reactive protein in the observation group were all lower the control group,withthe differences were statistically significant(P<0.05).The dyspnea and systemic condition scores in the observation group was significantly better than the control group,with the differences were statistically significant(P<0.05).Conclusion:Tolvaptancombined with small dose dopamine in the treatment of stubborn heart failure,the effect is significant.
