应用MPDS识别院外心脏骤停的敏感性及相关影响因素的研究

Sensitivity and Influence factors of detection out-of-hospital cardiac arrest by using medical priority dispatch system

目的 研究在使用医疗优先调度系统(MPDS)下对院外心脏骤停(out-of-hospital cardiac arrest,OHCA)识别的敏感性及其相关影响因素.方法 调度员使用MPDS受理呼救中确定为9-E代表心脏骤停代码的病例915例,通过回顾性调查方式,人工复听调度录音,进行统计分析.结果 MPDS识别OHCA敏感性42.51%,未进入MPDS程序27.1%,识别为其他的30.38%.识别OHCA的敏感性与调度员个人因素和患者年龄有统计学相关性(P<0.05),与调度员上班节律、患者性别无统计学相关性(P>0.05).结论 调度员个人因素是影响识别院外心脏骤停患者敏感性的主要因素,加强培训和考核,能够整体提高识别敏感性,为OHCA患者抢救补强生命链最初一环.

Objective To explore sensitivity dection and influence factors of out-of-hospital cardiac arrest(OHCA)by using Medical Priority Dispatch System(MPDS).Methods A retrospective study of manually re-listen to emergency calls and relative statistics was undertaken.Results Among the total 915 cases,42.51%OHCA cases were correctly detected and 27.1% didn't enter the MPDS.Percentage of 30.38%was incorrectly detected.Sensitivity of detection was stistically associated to dispatchers'personal factors and age of patients(P<0.05)instead of duty rhythm of the dispatchers and gender of patients(P>0.05).Conclusion Dispatchers'personal factors were mainly infuenced sensitivily deetion of MPDS.Training and evaluation should be reinforced to improve dection sensitivity which is impor-tant to timely save OHCA patients.
