不同麻醉方法对宫颈癌患者T淋巴细胞亚群和IL-6水平的影响

Effects of different anesthesia methods on T lymphocyte subsets and IL-6 levels in patients with cervical cancer

目的分析不同麻醉方法对宫颈癌患者T淋巴细胞亚群和IL-6水平的影响。方法选取2016年1月至2018年6月西安市中医医院诊治的86例宫颈癌患者作为研究对象。进行双盲随机分组,观察组患者(n=43)行硬膜外复合全身麻醉,对照组患者(n=43)行全身麻醉。分析两组患者行不同麻醉方法的手术指标,在手术结束后1天用流式细胞仪测定T淋巴细胞亚群和自然杀伤细胞(NK Cell)的比例,并通过ELISA法测定IL-6含量的变化。结果观察组患者的手术指标优于对照组,但差异无统计学意义(P>0.05);与对照者患者相比,观察组患者术后CD3^+、CD4^+、CD8^+和CD4^+/CD8^+比例均显著升高,差异均具有统计学意义(均P<0.05);与对照组患者相比,观察组患者术后NK细胞比例显著升高,IL-6水平显著降低,差异均具有统计学意义(均P<0.05)。结论对于行手术治疗的宫颈癌患者,相比单纯全身麻醉,硬膜外复合全身麻醉对患者细胞免疫功能和IL-6水平造成的影响较小,从而能够有效改善宫颈癌患者预后及生活质量。

Objective To analyze the effects of different anesthesia methods on T lymphocyte subsets and interleukin-6(IL-6)levels in patients with cervical cancer.Methods 86 patients with cervical cancer who were admitted to Xi'an Traditional Chinese Medicine Hospital were randomly divided into two groups,43 patients in the control group underwent general anesthesia,and 43 patients in the observation group underwent epidural combined general anesthesia.The surgical indexes of different anesthesia methods were compared between the two groups.At one day after the operation,the ratio of T lymphocyte subsets and NK cells was determined by flow cytometry,and the change in IL-6 content was determined by ELISA.Results The surgical indexes of the observation group were better than those in the control group,but the difference was not statistically significant(P>0.05);Compared with the control group,the proportions of CD3^+,CD4^+,CD8^+and CD4^+/CD8^+in the observation group were signifi-cantly increased(allP<0.05);Compared with the control group,the proportion of NK cell was significantly increased and the IL-6 level was significantly lower in the observation group(all P<0.05).Conclusions For patients with cervical cancer undergoing surgery,epidural combined general anesthesia has less effect on cellular immune function and IL-6 level than general anesthesia,which can effectively improve the prognosis and life quality of patients.
