体重指数、胰岛素抵抗在子宫内膜癌中的表达及其与预后的关系

Expression of BMI and IR in endometrial carcinoma and its relationship with the prognosis

目的研究体重指数(BMI)及胰岛素抵抗(IR)在子宫内膜癌患者中的表达及其与预后的关系。方法选取2016年3月至2018年3月蓬莱市人民医院诊治的100例子宫内膜癌患者作为研究对象。将这100例患者设为观察组,另选取同期进行体检的健康妇女100例设为对照组。分别对两组研究对象BMI、空腹血糖(FPG)、空腹胰岛素(FINS)水平进行检测。计算两组研究对象HOMA-IR,并分析BMI、HOMA-IR与预后的关系。结果观察组患者的BMI明显高于对照组研究对象,差异具有统计学意义(P<0.05)。观察组患者的HOMA-IR明显高于对照组研究对象,差异具有统计学意义(P<0.05)。随病情发展,患者BMI及HOMA-IR逐渐升高。Ⅲ期和Ⅳ期子宫内膜癌患者的BMI及HOMA-IR显著高于Ⅰ期和Ⅱ期子宫内膜癌患者,差异均具有统计学意义(均P<0.05)。多因素Logistic回归分析显示,BMI及HOMA-IR均为影响子宫内膜癌患者预后的危险因素,差异具有统计学意义(P<0.05)。结论子宫内膜癌患者的BMI及HOMA-IR普遍高于健康人,两者均为子宫内膜癌预后的危险因素,可影响疾病预后发展。

Objective To study the expression of body mass index(BMI)and insulin resistance(IR)in patients with endometrial cancer and its relationship with the prognosis.Methods 100 patients with endometrial cancer in Penglai People's Hospital from March 2016 to March 2018 were selected as the observation group.100 healthy women in our hospital for physical examination at the same time were selected as the control group.The BMI,fasting plasma glucose(FPG)and fasting insulin(FINS)levels of the two groups were detected.The insulin resistance index(HOMA-IR)of the two groups was calculated and the relationship between BMI,HOMA-IR and prognosis was analyzed.Results The average BMI index of the observation group was significantly higher than that of the control group,with statistically significant differences(P<0.05).The HOMA-IR of the observation group was significantly higher than that of the control group,with statistically significant differences(P<0.05).As the disease progresses,the patient's BMI and HOMA-IR gradually increased.The BMI and HOMA-IR of patients with stage Ⅲ and Ⅳ endometrial cancer were significantly higher than those of stage Ⅰ and Ⅱ endometrial cancer,with statistically significant differences(all P<0.05).Multivariate logistic regression analysis showed that both BMI and IR were risk factors for the prognosis in patients with endometrial cancer(P<0.05).Conclusions The BMI and HOMA-IR of patients with endometrial cancer are generally higher than normal,and both are risk factors for the prognosis of endometrial cancer,which may affect the prognosis of the disease.
