脂浊标本离心置换前后血红蛋白结果对比及临床价值

Comparison and clinical value of hemoglobin results before and after centrifugal replacement

目的探讨脂浊标本离心置换前后血红蛋白(Hb)结果对比及临床价值。方法收集我院2018年10月—2019年10月的30例血低甘油三酯水平的乙二胺四乙酸(EDTA)抗凝血液标本为对照组,另收集30例血高甘油三酯水平的EDTA抗凝血液标本为观察组。2组血液标本均进行低速离心生理盐水置换实验,乙二胺四乙酸二钾(EDTA-K2)抗凝管静脉采血2 mL混匀后2 h内完成全血细胞分析。比较2组脂浊标本低速离心生理盐水置换实验前后Hb测定结果,分析脂浊标本离心置换在Hb检测中的价值。结果生理盐水置换试验前,观察组Hb水平高于对照组,差异有统计学意义(P<0.05);生理盐水置换试验后,2组Hb水平均明显降低,差异有统计学意义(P<0.05),但组间Hb水平比较,差异无统计学意义(P>0.05)。结论脂浊标本会造成Hb检测结果假性增高,通过低速离心生理盐水置换可有效减少或消除脂浊对检测结果的影响,从而确保Hb检测准确性,具有较高的临床应用价值。

Objective To explore the comparison and clinical value of hemoglobin(Hb)results before and after centrifugal replacement of lipid turbid specimens.Methods Collected 30 patients with low triglyceride levels of ethylenediaminetetraacetic acid(EDTA)anticoagulated blood samples from October 2018 to October 2019 in our hospital as a control group,another 30 cases of EDTA anticoagulated blood specimens with high blood triglyceride levels were collected as the observation group.Two groups of blood samples were subjected to low-speed centrifugal saline replacement experiment.The EDTA-K2 anticoagulant tube blood was collected in 2ml and the whole blood was analyzed within 2 hours after mixing.Compared the results of Hb measurement before and after the low-speed centrifugal saline replacement experiment in two groups of lipid turbidity samples,and analyzed the value of centrifugal replacement of lipid turbidity samples in Hb detection.Results Before the saline replacement test,the Hb level in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05);After the saline replacement test,the Hb levels of the two groups were significantly reduced,the difference was statistically significant(P<0.05);However,compared in Hb levels between groups,there was no statistically significant difference(P>0.05).Conclusion Lipid turbidity specimens will cause falsely increased Hb test results.Low-speed centrifugal saline replacement can effectively reduce or eliminate the effect of lipid turbidity on detection,thereby ensuring the accuracy of Hb detection and having high clinical application value.
