同期经尿道手术治疗浅表性膀胱癌合并良性前列腺增生的临床疗效

Clinical efficacy of concurrent transurethral surgery for superficial bladder cancer accompanied by benign prostatic hyperplasia

目的:分析同期经尿道手术治疗浅表性膀胱癌合并良性前列腺增生的效果。方法:2018年7月-2019年10月收治浅表性膀胱癌合并良性前列腺增生患者96例,随机分为两组,各48例。对照组采用经尿道膀胱电切术治疗;研究组采取同期经尿道前列腺切除术治疗。比较两组治疗效果。结果:研究组手术时间、术中出血量、术后导尿管留置时间、住院时间及最大尿流速均高于对照组,差异有统计学意义(P<0.05);观察组残余尿量、前列腺症状评分(IPSS)及生活质量(QOL)评分均低于对照组,差异有统计学意义(P<0.05)。结论:同期经尿道膀胱电切术治疗浅表性膀胱癌合并良性前列腺增生手术出血量较多和术后恢复时间较长,但术后治疗效果明显优于单纯电切术,对提高患者生活质量有重要作用。

Objective:To analyze the effect of concurrent transurethral surgery on superficial bladder cancer with benign prostatic hyperplasia.Methods:From July 2018 to October 2019,96 patients with superficial bladder cancer accompanied by benign prostatic hyperplasia were enrolled,they were randomly divided into two groups with 48 cases in each group.The control group was treated with transurethral electrical resection of the bladder and the study group was treated with concurrent transurethral resection of the prostate.The therapeutic effects between the two groups was compared.Results:The time of operation,the amount of bleeding loss during operation,the time of indwelling catheter,the length of hospital stays and the maximum velocity of urine flow in the study group were all higher than those in the control group,the differences were statistically significant(P<0.05).The volume of residual urine,the prostate symptom score(IPSS)and the quality of life score(QOL)in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The treatment of superficial bladder cancer with benign prostatic hyperplasia by concurrent transurethral electrical resection of bladder has a long recovery time and more amount of bleeding loss during surgery,but the effect of postoperative treatment is obviously better than that of simple electric resection,which plays an important role in improving the quality of life of patients.
