骶神经调节术后手术部位感染相关研究的现状

摘要 手术部位感染是骶神经调节术较为常见的并发症,发生率较高,且严重影响疗效。此类感染最常见的致病菌为葡萄球菌属,甚至有一些为耐甲氧西林金黄色葡萄球菌,因此处理相对困难。术中囊袋深度过深、术后血肿形成是发生术后感染最可能的危险因素。仔细的体格检查对于明确诊断至关重要,同时推荐进行术区的影像学检查以排除积液可能。一旦发生感染,需要选择合适的抗生素进行及时、足量、足疗程的抗菌治疗。若抗菌疗法无法控制感染,需果断切开引流,必要时可以移除装置。若感染发生前疗效满意,可待装置移除术后6周~6月,切口完全愈合后,在对侧进行再次植入。建议有条件的医疗机构规范术前准备、术中操作和术后护理流程,最大程度避免感染的发生。本文综述了骶神经调节术后手术部位感染的概况及其危险因素、诊断、治疗预防等。
